What to do about side effects of pembrolizumab

The side effects of pabolizumab include myocarditis, hypertension, pneumonia, hepatitis, etc. Depending on the severity of the adverse reactions, measures such as temporary discontinuation and permanent discontinuation of the drug are available. Pabolizumab is a commonly used targeted therapy drug in clinic, which can be used in the treatment of some melanomas, non-small cell lung cancer, esophageal cancer, head and neck squamous cell carcinoma. Common adverse reactions after administration include pneumonia, colitis, nephritis, hepatitis, hyperthyroidism, pituitary gland inflammation, myocarditis, and hypertension. These adverse reactions are categorized into grades 0~5 according to their severity, of which grade 0 is no adverse reaction and grade 5 is death, so the clinical disposition is mainly for grades 1~4. For endocrine diseases such as hyperthyroidism and pituitary inflammation, no special intervention is needed for grade 1, grade 2 can be continued on the basis of drug substitution, and for grade 3~4 adverse reactions, the drug needs to be stopped until the adverse reaction recovers to grade 0~1 before continuing the drug. For pneumonia, colitis, nephritis, and hepatitis, no special intervention is needed for grade 1, temporary discontinuation of medication is required for grade 2, and permanent discontinuation of medication is required for grade 3 or higher. The use of the drug is prohibited in patients during pregnancy, lactation, drug allergy. Pabolizumab needs to be used under the supervision of a clinician.
