Abstract: Ovarian cancer, commonly referred to as ovarian tumor, is a common gynecologic tumor that can occur in women of any age. A 46-year-old female patient was deeply troubled by this disease. She reported that she had vague pain in the left lower abdomen without any cause in the past 1 week and came to our hospital for consultation.
Basic information】Female, 46 years old
Disease Type】Ovarian cancer
Hospital】Hunan Provincial Maternal and Child Health Hospital
Date of consultation】August 2021
Treatment plan】Surgical treatment (tumor cell reduction) + intravenous injection (paclitaxel injection + carboplatin injection)
Treatment period】3 weeks of hospitalization, regular chemotherapy, long-term follow-up
Treatment effect】Surgery went well, no serious adverse reaction to chemotherapy
I. Initial interview
When I first saw the patient in the hospital, the patient complained of sudden vague pain in the left lower abdomen in the past 1 week, which could not be relieved even after rest, and was not accompanied by abdominal distension, dizziness, and weakness. After listening to the patient’s description, I first performed a physical examination and found that there was a small amount of white thin discharge in the vagina, and a mass of about 6 cm in diameter was found in the left adnexal area with a hard texture, poor mobility and slight pressure pain. It was partially enveloped with irregular internal hypoechoic, partially dark fluid area separated by light bars and no significant change in posterior echogenicity. The patient was considered to be an ovarian tumor and was admitted to the hospital with the nature of pelvic mass to be investigated, and the next examination was performed after admission.
II. Treatment history
After the patient was admitted to the hospital, the pelvic MRI, tumor markers and other examinations were completed, and the possibility of ovarian malignant tumor was initially considered. After full communication with the patient, it was decided that a dissection would be performed as soon as possible, and a rapid frozen section would be sent during the operation to clarify the benign and malignant nature, and if the tumor was malignant, tumor cell reduction would be performed. After general anesthesia, a linear incision was made in the middle of the patient’s abdomen, and the left adnexa was removed and sent for rapid frozen section. The operation went smoothly and the patient returned to the ward safely after the operation. Combined with the pathological examination results, postoperative chemotherapy with paclitaxel injection + carboplatin injection was given, and genetic testing was recommended.
III. Treatment effect
On the day after the surgery, the patient’s mental status was fine and all vital signs were stable during the check-up. After 7 days of surgical treatment, the patient recovered well after surgery and was given paclitaxel injection + carboplatin injection after the pathological diagnosis was returned, with no adverse reactions. He was discharged from the hospital for convalescence and returned to the hospital for the second chemotherapy after 28 days, and insisted on long-term follow-up.
IV. Precautions
At the time of discharge, I told the patient to pay attention to rest, prevent colds and avoid heavy physical labor after going home. Blood tests should be repeated every other day and liver function tests should be repeated every week. She should come to the hospital for follow-up checkups at regular intervals so as to keep an eye on the progress of the disease and seek medical attention if there is any discomfort. At the same time, since ovarian tumor is a consumptive disease, pay attention to maintain a balanced nutrition and eat more high-protein and high-vitamin foods, such as fish, beef and green vegetables. Pay attention to rest and relax, do not be pessimistic and negative.
V. Personal insight
However, patients do not have to worry too much, as long as they insist on regular medical checkups and ovarian cancer screening, it can help to detect the disease at an early stage and provide timely treatment to inhibit the development of the disease. In this case, although the disease was not discovered during physical examination, the patient’s condition was controlled after surgery and medication, and she did not experience any significant adverse symptoms during treatment.
