[Composition] The bark contains cinnamon oil 1%-2%, the main component of the oil is cinnamylaldehyde (75%-95%), and contains a small amount of cinnamyl acetate, eugenol, cinnamon acid and so on. [Pharmacology] ① Cinnamaldehyde has obvious sedative and analgesic effects on mice. ②Cinnamaldehyde and sodium cinnamate have antipyretic effect. ③It can delay the time of tonic convulsions and death caused by stilbene. ④It has hypotensive effect on rats with adrenal cortical hypertension. ⑤ Cinnamon oil has a powerful bactericidal effect. It is more effective against gram-positive bacteria than gram-negative bacteria. It is rarely used as an antibacterial drug because of its stimulating effect. [Efficacy and Dosage] Nourishing Yuan Yang, warming the spleen and stomach, removing accumulated cold, and promoting blood circulation. It is used for the failure of vital fire, cold limbs and weak pulse, deficiency of Yang, abdominal pain and diarrhea, cold hernia and dolphin, cold pain in the waist and knees, menorrhagia and obstruction, gangrene, flowing injection, deficiency of Yang floating over and heat from above and cold from below. Internal use: 1-4,5g in decoction, 1-2g in powder. [Adverse effects] Small doses may cause motor inhibition and eyelid drooping; large amounts may cause strong spasms, dyskinesia, vasodilation and shortness of breath. Cinnamon oil 6-18g can cause death in dogs. Extra-conventional doses can cause red face, swollen eyelids, red tongue, little urine, and sensitivity to external stimuli. Inflammation and erosion of the gastrointestinal tract are seen after death. [Example] ①1 case of a 48-year-old male with “stomach pain” for 8 years, took 6g of cinnamon powder twice a day, 21 days later, distension and pain in the abdomen, little urination, red face and swollen eyelids, irritable heat and cold drinks, stool unresolved for 3 days, red tongue, yellow dry coating, urine protein +++), with tubular and red blood cells. (2) A 21-year-old female, after taking 10 whole tonic soup containing 18g of cinnamon per dose, after 16 doses, she was irritable and short of breath, her face was red like wine, her eyelids were swollen, frequent drinking did not quench her thirst, she had little urination and constipation, her tongue was red and dry, and her tongue was numb and her lips were cracked. [Prevention] ① This product is pungent, sweet and hot products, yin deficiency and fire is contraindicated, pregnant women should be careful. ② Once found that taking cinnamon poisoning should be immediately discontinued, while infusion, cast cold and nourishing Yin products to serve. [Source] This product is the sap of the stem of Phyllostachys nigra ( Lodd ,) Munro var , henonis Stapf ex Rendle, which is roasted by fire. [It contains more than 10 kinds of amino acids, as well as glucose, fructose and sucrose. [Pharmacology] With cough expectorant effect. [Efficacy and Dosage] Clearing heat and slipping phlegm, relieving fright and relieving orifices. It is used for stroke and phlegm confusion, lung-heat and phlegm congestion, frightening wind, epilepsy, strong heat and thirst, child annoyance, and tetanus. Dosage: 30-60g. [Adverse Reactions] May cause gastrointestinal bleeding. [Example] A 70-year-old woman took 30ml of fresh bamboo syrup twice a day for cough and sputum. 30 days later, she suddenly developed abdominal pain, vomited about 100ml of coffee-colored liquid, black stool, and occult blood in the stool (+). After stopping the fresh bamboo syrup and treating with cimetidine for 5 days, the stool turned yellow. Six months later, the cough was treated with 30ml of fresh bamboo leech syrup twice a day, which was stopped after 1 week because of epigastric pain. 2 days later, the abdominal pain disappeared. 1 week later, the patient continued to take fresh bamboo leech syrup again, and 5 days later, the abdominal pain was severe, vomiting a lot of coffee-colored liquid and black stool. [Prevention and control] Bamboo leech is cold and slippery, if cold and damp stomach deficiency intestinal slippery people take it to hurt the stomach and intestines, the old and weak should be used with caution. Once the abdominal pain occurs, the medicine should be stopped immediately and treated symptomatically. If you have gastrointestinal bleeding, treat as gastrointestinal bleeding. Also avoid using it again in the future. References 1. Liu Shoushan, Abstract of Chinese medicine research literature 1980 -1984, Beijing: Science Press, 1992, 370, 371 2. Zhusha Cinnabaris [synonym] Dansu (Shanhaijing), Dansha (Benjing), Red Dan. Ben Jing (Ben Jing), Red Dan (Huai Su Zi), Mercury Sand (Shi Yu Er Ya), Cinnabar (Ben Cao Tu Jing). [Source] It is natural mercury sulfide ore. [The main component is mercuric sulfide, which is equivalent to HgS in pure form, but it is often mixed with stannum and phosphoric lime. [Pharmacology] ①It has astringent and hardening effect. ②It has antiseptic and antiseptic effect. [Efficacy and Dosage] Tranquilizing, calming, brightening eyes, detoxifying. It is used for epilepsy, palpitation, heart trouble, insomnia, dizziness, dizziness of the eyes, swelling and poison, sores and scabies. For internal use: Powdered 0,3-0,9 g. For external use: Combined with other medicines, powdered and sprinkled dry. It should not be taken for a long time or more than one dose. [Adverse Reactions] Mainly a manifestation of mercury poisoning. In mild cases, swelling or decay of the mouth and throat, swelling of the lymph nodes in the neck, marked increase of saliva secretion in the mouth, difficulty in swallowing, headache and fever, palpitation and insomnia, fear, constriction of the limbs, rash. In severe cases, severe abdominal pain, blood in urine, little urine, respiratory distress, tiny pulse, decreased body temperature, and finally renal failure and heart failure and death. [Example] ① there is a case of 70-year-old female, due to palpitations take vermilion 1-1, 5g, 3 times a day, 30 days accumulated 100g. appear abdominal pain, diarrhea, vomiting black watery material, coffee-colored stool, blurred vision, oliguria 24h, anuria 9h, urea nitrogen 50, 5mg%, after diuretic, anti-inflammatory, detoxification rehydration multiple resuscitation ineffective death. ②There are 2 cases of taking vermilion causing dizziness and headache, muscle tremors, and gums see purple-blue mercury lines. (③There were 2 cases of chronic poisoning with gradually increasing dizziness and headache, numbness of the scalp, anthrax sensation, nausea, bleeding from the dental stuff, and abdominal discomfort. In one case with red hair, the hair was red, starting from the roots and progressing to the tips, and the pubic hair and axillary hair became red and yellow, but the fine hair did not become red (the body was healthy and black in the past). ④ There was a case of a 59-year-old woman who took vermilion with white wine for the last 3 years, 10-20 g each time. developed dizziness and headache, progressive numbness and weakness of the limbs, muscle tremors in both lower limbs to bilateral lower limb paralysis, and urinary incontinence. After various examinations, it was found to be vermilion poisoning causing myelopathy. ⑤ There was a case of a 53-year-old female who overdosed on 3g of vermilion due to arrhythmia and blended it with decoction. When taking the 7th dose, palpitations increased, dyspnea, vomiting, followed by cardiac arrest, and died after resuscitation. (6) A 66-year-old male, due to palpitations and insomnia for half a month, bought 20g of vermilion with decoction from a pharmacy and took it by himself; then bought 30g of vermilion and took 40g of vermilion in 5 days, after which he developed nausea and vomiting, little urine, 24h urine volume about 200ml, urine protein (++), RBC ( + ), tube type (+ ) , BUN 24, 1mmol/L, Cr345μmol/L. The diagnosis was mercury poisoning. [Prevention and control] ① This product is toxic, and after oral absorption, it stays in the body for a long time and is excreted slowly, with the possibility of accumulation. Therefore, Chinese medicinal preparations containing vermilion should not be taken in large quantities for a long time. The dosage should be limited to 0.3-0.9g for each internal dose, and should not be taken continuously for more than 5-7 days, otherwise mercury poisoning may occur. ② If a large amount of vermilion is taken at one time, the stomach should first be lavaged with 2%-5% sodium bicarbonate, or 0.5% edetate disodium solution. After gastric lavage and then oral milk, soy milk or protein food to further reduce the absorption of mercury. At the same time appropriate oral magnesium sulfate, play a laxative role to promote the excretion of mercury. Further, 1-2g of sodium phosphate plus 1g of sodium acetate dissolved in 250ml of water can be taken orally in 4-6 doses, once every hour. It can reduce the divalent mercury compound to monovalent mercury compound, and the toxicity to human body is reduced. ③ Each internal dose is within the therapeutic amount, but if the symptoms of mercury poisoning appear after more than 10 days of continuous use, the first thing to do is to stop taking vermilion (or drugs containing vermilion) and start detoxification and mercury expulsion treatment. Sodium dimercaptopropionate 5% solution, 5ml each time, twice daily, for 3 consecutive days, with an interval of 4 days as a course of treatment. If needed, treat for another 1-3 courses. d-penicillamine hydrochloride, orally 0,03g each time, 3 times daily for 1-2 weeks. Allergy test should be done before use. During the treatment period, attention should be paid to the prohibition of salt to avoid increasing the solubility of ascending mercury. When toxic nephropathy causes oliguria and urinary closure, it should be treated as acute renal failure. If a large amount of internal consumption causes abdominal pain, pepper can be ground and swallowed to make the mercury excreted from the stool. For stomatitis, 10g each of Guanzhong and Huanglian can be used to wash the mouth with decoction. For joint and muscle pain throughout the body, use several pounds of pepper and fry it, and lay it on the bed. A decoction of mung bean, orris and licorice can be used internally for detoxification. Used for pain in the heart, abdomen, waist and knees, menstrual disorders, obstruction in the abdomen, postpartum dizziness, incomplete dew, bruises and injuries. Internal use: 4,5-9g in decoction. [Adverse Reactions] Yanhuosuo has low toxicity and the general dosage has no significant effect on heart rate, blood pressure and liver and kidney function. Occasionally, nausea, dizziness and weakness may occur. However, respiratory depression and manifestations of Parkinson’s syndrome may occur in overdose. Individual patients have skin allergy. [Example] A 32-year-old woman with painful left upper limb phlebitis applied Yanhuzo infusion (prepared by the hospital preparation room) to relieve pain. About 30 min after application, the patient became generally unwell, nauseated and dizzy. 1h later, the symptoms were significantly aggravated, followed by chest tightness, shortness of breath, palpitations, numbness of the lips and end extremities, convulsions, and flushed and itchy skin all over the body. After 10h of symptomatic anti-allergic treatment, the patient returned to normal. On the next day, the above allergic phenomena occurred again after 30 min of application of Yanhuo infusion. [Prevention and control] For those with allergic reactions, the drug should be stopped immediately, rehydrated promptly to help excretion, and avoided next time. (2) A patient with itching around the body, flushing of the skin on the extremities and back of the chest and receding under pressure, marked angioneurotic edema in the hands and feet, and dozens of bean-sized blisters after taking internal blood exhaustion. Shortness of breath. [Prevention and treatment] Clinical internal administration cannot be considered completely risk-free. Allergy tests (e.g. patch test) may be performed first if necessary. If there is contact allergy, contact should be stopped immediately and active anti-allergy treatment should be given after allergy occurs. Cai Yunying, et al. 2 cases of contact sensitization to blood dried scorpion, Chinese Journal of Traditional Chinese Medicine, 1995, 20(1):57 63, Quanxie Scorpio [synonym] Quanxie Scorpio (Chinese Traditional Medicine Shape Experience Identification Method), Porcupine Scorpion (Shanxi Traditional Chinese Medicine) [source] This product is the dried body of the East Asian scorpion Buthus martensii Karsch. Dried body. [It contains scorpion venom (katsutoxin, similar to snake venom neurotoxin), trimethylamine, various enzymes (such as phospholipase A2, etc.), histamine, 5-hydroxytryptamine, etc. [Pharmacology] ①Anti-convulsant effect: “Kai Bao Ben Cao” said that this product “cure all wind addiction rash, …… hand and foot twitching.” ②Cardiovascular effects: In animal experiments, intravenous injection of the decoction of this product can cause a sustained reduction in blood pressure 1-3h, some species of scorpion toxin can occur hemorrhagic changes and local necrosis. ③On skeletal muscle: Some scorpion toxins have direct effects on muscles, causing twitching and tonic spasms. ④Several scorpion toxins can cause elevated blood glucose in rats. [Efficacy and dosage] Used for the treatment of epilepsy and paralysis. It is often shared with Centipede and swallowed as a pill or a powder. Common dosage: 2-5g. [Adverse Reactions] Salivation, contraction paralysis of limb muscles, followed by respiratory paralysis occurred after injection of scorpion venom into animals. The severity of symptoms after scorpion chelation in humans is related to age, and the deceased are mostly children and young people. It is also related to the scorpion species: less toxic scorpions have only local reactions, while more toxic ones can produce systemic symptoms. Local reactions are swelling, fever, burning sensation, and pain with numbness, which decreases after a few hours. Systemic reactions include sleepiness, chills, dizziness, recovery in about two to three days, sometimes hyperglycemia and pancreatitis. In very severe cases severe diarrhea, pulmonary edema, and shock [1]. Whole scorpion is less toxic than the aforementioned for oral use, but can still have adverse effects. [Example] ① Caused respiratory depression in neonates: 3 neonates 2-10 days after birth contained 1 g of whole scorpion in each dose of the soup, and 6 h after the second dose developed drowsiness, non-suckling, superficial breathing, nasal flapping, cyanosis of the lips, cold extremities, and a weakened sucking reflex. After admission, the child was treated with tachyphylaxis 3mg, niclosamide (coramin) 0,1lmg in saline intravenously, then 5% sodium bicarbonate 15ml in 5% glucose intravenously, after 4h the child recovered response to stimulation, would cry, could suckle after 6h, and was cured in 2 days [2]. (ii) Caused proteinuria, a male patient with cerebral thrombosis, 48 years old, no previous history of renal disease, no abnormality in urinalysis on admission, lumbar discomfort after taking a formula containing 6 g of whole scorpion per post, lumbar pain aggravated after taking the second dose, dyspareunia, protein in urinalysis (+++), lumbar pain alleviated after removing whole scorpion from the formula, and protein in urine also disappeared [3]. (iii) Causing allergic reactions:A 68-year-old female patient was itching all over her body after taking a formula containing whole scorpion, and a red rash appeared on her neck, chest and limbs, which disappeared after stopping the medicine. Half a month later, because the pain of herpes had not decreased, 6 g of whole scorpion powder was used and taken in two doses, and itching and red rash occurred that night, which was confirmed as allergy to whole scorpion [4]. Zhang’s also reported a similar case [5]. Zhao reported that after taking a formula containing centipede and whole scorpion for ear disease, a patient not only had itching and rash, but also had painful red eyes, shame, swollen blisters like peaches, and corn-like pus spots on the eyelids, accompanied by chest tightness, fever, poor appetite, yellow urine, and dry stools [6]. [Prevention] The dosage should be controlled, according to the dosage for children does not exceed 1/6 of the adult amount, then the aforementioned case of neonatal respiratory depression 1 g per day is a risk of excessive dosage. In addition, those who are allergic to whole scorpion should stop the drug and make symptomatic treatment. The first aid after being wounded by a scorpion cheek is to immediately pull out the poisonous stinger, tie the wound proximally, squeeze out the venom, clean with 1:5000 potassium permanganate solution to remove the remaining poison. 3% ammonia or 5% sodium bicarbonate solution can be applied externally to relieve pain, and 0.25% procaine local seal can effectively stop the severe pain. If systemic symptoms appear, intravenous rehydration, intravenous calcium gluconate 10%, intravenous hydrocortisone 100mg, intramuscular atropine 1-2mg, Chinese herbal medicine such as Nantong snake medicine, Wan Ying ingot etc. local application also have good effect. Recently, Karnad et al. treated severe scorpion toxin poisoning with oral Captopril (CPT) 12 and 5 mg during infusion, and the pulmonary edema and hypotension were relieved [7]. References 1. Internal use: 3-6g. [Adverse Reactions] Initial discomfort in the upper abdomen, followed by headache, chest tightness, nausea and vomiting, abdominal distension and pain, diarrhea, swelling, dysuria, urinary frequency and urgency, nocturia, inability to lie down, confusion, and finally death due to acute renal failure. [Example] ①A case of a patient with lumbar and leg pain, after taking 60g of single mucuna pruriens, developed abdominal distension and pain, nausea and vomiting, oliguria and even anuria, and eventually died of renal failure. (②One case of using only adzuki beans cooked with mucuna pruriens caused abdominal pain and diarrhea, swelling, decreased urine output, and metabolic acidosis. ③A farmer mistakenly used 120g of mucuna pruriens due to dizziness and insomnia, steamed and consumed, developed general weakness, decreased urine output, swelling of both lower limbs the next day, frequent vomiting, acute renal failure and uremic toxicity, and died 9 days later. ④ 1 case of rheumatoid arthritis took 50g of mucuna pruriens in water for 2 times, and 15g of mucuna pruriens in water for the next day, and then urine volume decreased after 2 days, which became worse day by day, and on the 6th day there was frequent eruption and vomiting, and on the 8th day urine volume decreased to 50ml per day and he was admitted to hospital. Physical examination: fatigue, slightly distended abdomen, urine protein (++++), leukocytes (+), red blood cells 0-2, tubular type (++++). Diagnosis:Acute renal failure (due to mucuna pruriens poisoning). Upper gastrointestinal bleeding appeared on the 6th day of admission, ascites appeared on the 10th day, and then he was discharged home due to economic failure, and 4 months later, he died due to uremic toxicity. ⑤ 1 case of arthralgia purchased 100g of Mucuna pruriens, 6g of Fanfeng, 6g of Qiangwu decoction, 2h after taking it, he felt epigastric discomfort, accompanied by nausea and vomiting, the next day, appetite greatly reduced, urine output decreased, acute renal failure, after rescue treatment failed, died 25 days after admission). (6) Six cases of mucuna pruriens poisoning, symptoms of abdominal cramps, anorexia, vomiting and diarrhea, black stool, urine, proteinuria, and finally all died due to renal failure. [Note] The Dictionary of Traditional Chinese Medicine states that Mouton is “very toxic when taken orally”. Some other medical texts also do not point out that Moutong is toxic, so it is easy to be misused in large quantities. It is clinically proven that Mouton is not non-toxic, so it must be used with caution, should not be used in large quantities, and should not be taken for a long time, especially for the elderly and people with reduced kidney function. The usual dosage of 3-6g is appropriate. References 1. Zhang Junwen, et al. Report of 20 cases of poisoning by traditional Chinese medicine, Journal of Xi’an Medical University, 1986,7(2):176 2.
