With the promotion of double eyelid surgery, the incidence of postoperative double eyelid asymmetry has increased. Asymmetrical eyelids cause damage to the patient’s appearance as well as different degrees of psychological damage. Surgery is the main means of repairing eyelid asymmetry and is mainly individualized according to the specific cause of the eyelid asymmetry. The causes of eyelid asymmetry include different skin tension between the two sides when marking the eyelid line, incision deviation from the marking line due to inaccurate measurement or unskilled operation, and different heights of the skin fixed to the lid plate on each side. This can lead to a significant difference in the shape of the lids, which can affect the intraoperative judgment. The solution is to perform a new blepharoplasty for those who are not satisfied bilaterally. For those who are relatively satisfied with one side of the lid, blepharoplasty can be performed on the unsatisfactory side according to the ideal shape.
