Recently, people often ask me that there are too many friends around me who have stomach cancer, which is one of the malignant tumors with the highest incidence rate worldwide, and China is a high incidence area. According to the data, there are 400,000 new gastric cancer patients and 300,000 deaths in China every year, and about 90% of them are in the middle and late stages when they are diagnosed. In recent years, there is a trend of youthfulness, and the number of young people suffering from stomach cancer in China has doubled compared with 30 years ago. On average, there are 753 Chinese people suffering from upper gastrointestinal tumors every minute. At present, the etiology of gastric cancer is still not completely clear, and many factors such as gastric cancer are related to it, such as living habits, types of diet, environmental factors, genetic quality, mental factors, etc. Some diseases such as chronic gastritis, gastric polyps, and Helicobacter pylori infection in the stomach are all related to the occurrence of gastric cancer, thus early detection, early diagnosis and early treatment are the keys to keep away from gastric cancer. Unfortunately, in the early stage of gastric cancer, there are no or only mild symptoms, which often do not attract people’s attention and are treated as “indigestion”, such as mild abdominal discomfort, belching, heartburn, acid reflux and abdominal distension, which are difficult to be distinguished from chronic gastritis and gastric ulcer. When the tumor develops to advanced stage, chronic abdominal pain, vomiting, difficulty in swallowing, vomiting blood, blood in stool, weight loss, loss of appetite, anemia, etc., and sometimes a small hard lump in the left side of the neck may be found, which are the manifestations of advanced gastric cancer. First of all, we should pay high attention to it and develop good living habits. When there is discomfort in upper abdomen, check it early. In fact, this is quite dangerous because stomach diseases, such as gastritis and gastric ulcer, will be relieved when taking some drugs, especially drugs to reduce gastric acid secretion, but stomach tumors, especially ulcer type tumors, will also be relieved when taking these drugs. However, tumors of the stomach, especially ulcerated tumors, may also experience symptom relief when taking these drugs, which may give the illusion of “cure” and lead to delay of the disease. In clinical practice, we often encounter patients with advanced gastric cancer who take drugs for a long time, even for a year or several years, because their symptoms are relieved after taking the drugs, resulting in the tumor missing the best treatment period and cannot be removed, which is regrettable. Therefore, what examination should be done when there is stomach discomfort? The most common clinical examination methods are barium meal examination of upper gastrointestinal tract and gastroscopy, both of which are complementary to each other. Since biopsy of the lesion can be performed at the same time as gastroscopy, the accuracy of the latter examination is high, up to 98%. In fact, with the progress of technology, the new electronic gastroscope is no longer the same as before, its mirror is thin, soft, good compliance, coupled with the doctor’s proper operation, the examination can be completed in a few minutes, the patient only feels some nausea, and there is not much pain. The patient only feels a little nauseous and does not feel much pain. In recent years, with the development of painless gastroscopy, the patient can complete the examination in a painless “sleep”, so there is no need to worry. It is not difficult to diagnose stomach cancer, but the key is to pay attention to it. Therefore, when symptoms such as upper abdominal discomfort, vague pain, feeling of fullness after eating, loss of appetite, emaciation, weakness, and unexplained blood in stool and black tar-like stool occur, they should be highly valued and should go to regular hospitals for regular examination first, and then regular treatment should be carried out after the cause is clear, and should never be delayed. For those who have chronic atrophic gastritis, gastric ulcer, gastric polyp, pernicious anemia, post-gastrectomy and family history of gastric cancer, they should have regular gastroscopy, which is recommended once every six months or once a year. The treatment effect of gastric cancer is closely related to the early and late detection, and surgery should be preferred for the treatment of gastric cancer. Research shows that early gastric cancer can be completely cured by surgical resection, even without post-operative chemotherapy and radiotherapy, while the more advanced it is, the worse the treatment effect is, such as clinical stage IV cancer, the five-year survival rate after surgery is less than 10%, which shows how important early detection is.
