No obvious abnormality was found in the magnetic resonance examination of the head, and Bell’s palsy was considered, so she was admitted to the hospital for treatment. The patient was first given nutritional neurological, hormonal, and antiviral symptomatic support, and then given acupuncture and rehabilitation treatment, and recovered well. Basic information] Female, 27 years old [Type of disease] Bell’s palsy [Hospital] The Second Affiliated Hospital of Harbin Medical University [Date of consultation] February 2022 [Treatment plan] Medication (acyclovir tablets + methylcobalamin injection + glyceryl fructose injection + Vitamin B1 tablets + Methylprednisolone sodium succinate for injection) [Period of treatment] Hospitalization for 7 days [Effect of treatment] Decrease in the sensation of taste and auditory symptoms have been alleviated. The patient reported that he felt that the front part of his tongue had no taste when he ate two days ago, thinking that it was caused by cold and flu, and he did not see any obvious relief after taking cold and flu medicines orally, and then he had the symptom of feeling abnormally loud when listening to very small voices, and then he came to our outpatient clinic. The patient was given a brief examination: clear consciousness, fluent speech, body temperature of 36.5℃, blood pressure of 115/80mmHg, heart rate of 74 beats/min, respiration of 16 beats/min, right peripheral facial paralysis, limb muscle strength is normal, auditory hypersensitivity. Considering the possibility of Bell’s palsy, the general problem is not big, can be hospitalized and treated with medication, informed the patient expressed acceptance, and then admitted to the hospital for treatment. After admission to the hospital, the patient underwent magnetic resonance examination of the head, which showed small ischemic foci in the brain, and at the same time, blood was collected to check routine blood tests, coagulation, troponin, cardiac enzymes, liver function, renal function and other laboratory tests. Based on the results of the examination, a clear diagnosis of Bell’s palsy was made. The patient was told that as the condition was not serious, he could basically recover as long as he cooperated with the treatment, and the patient expressed peace of mind and willingness to follow the doctor’s instructions for active treatment. The patient was then given acyclovir tablets for oral antiviral treatment, methylcobalamin injection for nerve nutrition, glycerol fructose injection for relief of facial nerve edema, vitamin B1 tablets for oral nerve nutrition, and methylprednisolone sodium succinate injection for reduction of inflammatory reaction. Third, treatment effect The patient is young, so there is much room for recovery. After 7 days of treatment, the body recovered well. The patient complained that the symptoms of hyposmia and auditory hypersensitivity had been relieved, and she could taste sour and sweet food, and her speech did not seem so loud, and the symptoms of crooked mouth were slightly relieved, so she was discharged from the hospital. Before discharge, the patient was instructed to follow the doctor’s instructions to continue the application of drugs and gradually reduce the amount, but also to the rehabilitation hospital for treatment. Precautions We are glad to see that the patient’s discomfort gradually disappeared after active treatment, and his body regained its healthy state. It is recommended that the patient should start the functional training and rehabilitation therapy as soon as possible after discharge from the hospital. He can close his eyes, frown, show his teeth, whistle, puff his cheeks, etc., several times a day for about 10 minutes each time, assisted by facial massage; he should eat less spicy food and less stimulating food, such as chili pepper, ginger and garlic. After being discharged from the hospital, the patient can go for rehabilitation treatment with ultra-short-wave diathermy therapy. The patient was a typical Bell’s palsy patient, after applying antiviral drugs and nutritive neurological drugs, the patient’s prognosis was good. Patients with Bell’s palsy can recover in a few weeks to 2 months, taste often precedes the recovery of motor function, such as 1 week taste recovery suggests that the prognosis is good, incomplete facial paralysis can be recovered in 1-2 months, the prognosis of young patients is good, the prognosis of older patients with cardiovascular and cerebral vascular complications is poorer. After diagnosed with Bell’s palsy, patients should follow the doctor’s instructions for treatment. In addition to adhering to the medication, attention should be paid to functional training and rehabilitation therapy to promote recovery.
