The clinical treatment of gouty arthritis aims to rapidly control the acute inflammation of the joints and prevent recurrence of arthritis. Prevention or treatment of complications caused by deposition of urate crystals in the joints, kidneys or other tissues. According to the above treatment objectives, there are several treatment measures as follows. 1.Dietary treatment: Limit the intake of high purine food; limit the high-calorie and high-protein diet and control the total amount of diet; eat more alkaline food and drink more water to promote uric acid excretion. 2.General treatment: First of all, diseases associated with gout should be actively treated, such as high blood pressure, hypertension and diabetes, etc. Obese people should reduce their weight. Try to avoid promoting factors, such as alcohol consumption, mental planing injury, infection, excessive fatigue and the application of certain drugs. Such as sulfonamide and insulin, etc. Frequent heavy drinking can lead to high blood uric acid and high blood lactic acid, therefore, gout patients should quit drinking. 3, drug treatment: ① control of arthritis drugs: the preferred drugs for non-steroidal anti-inflammatory drugs, such as fotarine its rapid and significant effect; or chinolig or anti-inflammatory pain or naproxen, ibuprofen, etc.. Colchicine is the traditional effective drug for the treatment of gouty arthritis, but the side effects are large. ② drugs to reduce blood uric acid: the indications for the application of such drugs are urinary stones, gout stones, arthritis with markedly elevated blood uric acid and frequent attacks. I. Probenecid: Probenecid is an effective uric acid-removing drug. The principle of clinical use should be to start with small amounts. Slowly increase and maintain with the minimum effective amount. To prevent deposition of the drug in the kidney, more water is required. Hydroyanic acid drugs have an antagonistic effect on propofol and should not be taken simultaneously. Propofol may increase the accumulation of uric acid in the body in cases of renal dysfunction and should be used with caution. II. Gout Lixin: Gout Lixin, also known as benzbromarone, increases uric acid excretion by inhibiting the reabsorption of uric acid in the kidneys. Blood uric acid can be reduced to normal in 1 week of treatment. III. Allopurinol: Allopurinol can inhibit uric acid production. Bone marrow suppression and liver damage can occur with allopurinol and should be closely monitored. 4. Surgical treatment: Gouty stones can be removed surgically, but care should be taken not to cut the tendons. For gouty arthritis of knee and ankle, arthroscopic flushing can be used, and our department has treated many cases with satisfactory results.
