Normal female physiology: The normal size of a woman’s vagina is approximately the same diameter or slightly smaller than the cervix, which is 2.5 cm in diameter and fits into two fingers. The vagina is a muscular canal that is used for sexual intercourse, menstrual bleeding, and delivery of the fetus. The outer 1/3 of the vagina and the mucosa surrounding the vaginal opening are rich in sensory nerve endings, and the vagina’s force on the penis is mainly generated by the muscles of the outer 1/3 of the vagina, so vaginal tightening should focus on the repair of the muscles of the outer 1/3 of the vagina. The outer 1/3 of the vagina is also known as the love muscle, which is the skeletal muscle that surrounds the lower part of the vagina rather than the smooth muscle that makes up the vaginal canal itself. From superficial to deep they are: 1. superficial perineal muscle – the vaginal sphincter (which narrows the vaginal opening) starting from the central tendon of the perineum; innervated by the parasympathetic pelvic visceral nerve (also called erectile nerve); 2. deep perineal muscle – the urethrovaginal sphincter located in the deep transverse perineal muscle sphincter (which tightens the urethra and vagina); 3. anal raphe – the pubovaginal muscle in the front and the puborectalis muscle in the middle (which makes the vagina contract and is the main muscle that produces grip force). When the tone of the above pelvic muscle groups decreases, it can cause relaxation of the muscles around the vagina and widen the diameter of the vagina to 3-4 fingers. In postpartum women, vaginal laxity can be caused by vaginal delivery and lateral perineal suturing, which can lead to decreased sexual sensation and uterine prolapse, increasing the psychological pressure on both spouses. In older women, as time passes, in addition to laxity of the skin of the facial ligaments, degenerative changes in the structures supporting the female pelvic floor can occur. Cosmetic female genital surgery began to appear in 1978, and vaginoplasty revision surgery includes the vaginal opening, the caliber of the vagina, and the epithelial tissue of the vagina. In addition to the aesthetics of the vulva, plastic surgeons and gynecologists focus on vaginal laxity or the patient’s own perception of vaginal laxity or reduced friction during sex, with the goal of improving the satisfaction of the couple during sex. For this reason, surgical procedures can be performed to repair the anterior vaginal wall and to repair the high posterior vaginal wall, removing or folding the vaginal mucosa layer. In recent years, surgery has revealed that excision of the vaginal lateral wall or the anterior and posterior vaginal vault mucosa can reduce the diameter of the vagina, giving a surgical result of tightening and changing the lax vagina. After vaginal tightening, you need to perform Kegel (Kegel) exercises, which is a conscious contraction exercise mainly for the anal levator muscle for at least 6-8 weeks, as follows Lie on your back and relax your body, then contract the vagina. You can insert two fingers gently into the vagina if the fingertips feel pressure, which means that the contraction is effective, but there are also special instruments for training instead of fingers. At the same time, place the other hand on the abdomen and sense if the abdominal muscles are relaxed. Avoid contracting the abdominal muscles and gluteus maximus, but focus on contracting the muscles around the vagina and anus. 2. The cause of vaginal laxity can be congenital or due to transvaginal delivery, old perineal tears, and poor wound healing after lateral perineal incision.
