When is the best time to operate for glaucoma?

    Any tissue damage to the eye caused by glaucoma is irreversible. In general, IOP-lowering therapy should be started when the IOP is elevated enough to cause damage to visual function. Topical spot ophthalmic treatment is possible for early patients, but for patients with closed-angle glaucoma, good results can be obtained with early peripheral iridotomy or laser iridotomy when the atrial angle is mostly open in the early stages. Some glaucoma is not easily controlled or often recurring, and during the examination, the color of the optic nerve in the fundus is found to be lighter, the cup/disk ratio is larger, and the visual field is defective or the optic nerve fiber layer is found to be defective in fundus photography, etc. This means that the medication can no longer control the IOP, which is an indication for surgery. A large proportion of patients with glaucoma are no longer in the early stage and should be treated with surgery as soon as possible.
