I. Initial consultation
The patient is male, 40 years old. He reported that he had a mild throat discomfort soon after the renovation of his home started, but did not have any special treatment. About one month ago, he felt pain in the pharynx, dryness and itching in the pharynx, and a foreign body sensation, but no cough and sputum, no headache and dizziness, no nausea and vomiting. He was treated with intramuscular medication (details unknown) at the local health office, but his symptoms were reduced, but he still had recurrent episodes. The patient had a history of diabetes mellitus and was taking metformin hydrochloride and glimepiride tablets orally, and his blood glucose level was well controlled.
II. Treatment history
The patient cooperated with the perfect blood routine examination, which indicated elevated white blood cells. Considering that chronic tonsillitis was considered to be affected by chronic nasopharyngitis, so the inflammation should be controlled first, and after communicating with the patient and his family, it was decided to take medication. The patient was given intravenous penicillin sodium for injection to kill the bacteria and control the inflammation; topical inhalation treatment with injectable erythromycin hydrochloride to relieve nasal and pharyngeal discomfort; and because the patient’s pharyngeal pain was more obvious, he was given acetaminophen tablets for pain relief and stopped taking them after the pain was reduced; in addition, metformin hydrochloride tablets and glimepiride tablets were taken normally. Glimepiride tablets were taken normally.
III. Treatment effect
After 3 days of taking painkillers, the patient found that the congestion in the pharynx was reduced, so the patient was instructed to stop taking the drugs and to continue using the rest of the drugs. On examination, the external redness and swelling of the nose disappeared, there was no follicular hyperplasia in the pharynx, and the tonsils were not enlarged bilaterally, indicating that the treatment plan was effective and the patient’s condition was under control.
IV. Notes
After the treatment, the patient’s discomfort disappeared and her face was smiling again, and I was happy for the patient’s recovery. At the same time, I reminded the patient that chronic rhinopharyngitis is very prone to recurrence, so even if the home renovation is not completely finished, it is recommended that the patient should not go in and out of the home renovation site unless it is necessary, so as not to stay in a space with more dust or irritating gases for a long time, which may trigger the disease again. Even if you go to the renovation site, you should wear a mask and open the windows to reduce the stimulation of dust and other substances on the local mucous membrane; in addition, although the disease has been cured, you still need to check the pharynx and nasal cavity every 3 months for abnormalities.
V. Personal insight
Chronic nasopharyngitis is not only related to infection, but also similar to the patient in this article, because of the stimulation of dust and irritant gas, which induces local inflammation, so patients should reduce the time of long-term stay at construction sites; in addition, in spring, windy weather, hazy days, etc., patients are advised to wear masks to reduce the inhalation of dust; finally, in daily life, we should also keep indoor air circulation, weather When the weather is dry, you can add a humidifier to keep the indoor air moist and reduce the chance of recurrence of chronic nasopharyngitis.
