Causes and countermeasures of urinary frequency and urgency in middle-aged and elderly men

   The above situation is actually a manifestation of benign prostatic hyperplasia (BPH), a disease characterized by progressive enlargement of the prostate gland causing obstruction of urinary discharge from the bladder due to reduced testicular function and epithelial hyperplasia, a common condition affecting the quality of life and health of middle-aged and elderly men. This disease is mostly seen in men over 55 years old.  According to Chinese medicine, the normal physiological urination process is achieved by the coordination of the following organs.  The body takes in food and water, and through the transportation of the spleen, the essence is absorbed to produce blood, fluid and semen, which are transported to the tissues of the whole body; the waste fluid produced by metabolism is transported down to the bladder by the spleen’s function of transporting water and moisture to form urine and excrete out of the body, so it is called “the kidney is the master of water”. In addition, the kidney and the bladder are in close proximity to each other, and kidney qi plays an important role in participating in bladder gasification and regulating the opening and closing of the bladder. Kidney yin and kidney yang work together to produce marrow, which is connected to the brain (heart), the house of the spirit, and the process of urination requires mental activity to control and regulate. The bladder stores urine to a certain volume, and the pressure stimulus generated is accepted by the “heart” of the master spirit, forming a physiological feedback between the heart and the bladder.  As we age, the kidney energy declines, the kidney yang energy is insufficient, no power to consolidate, bladder gasification weakness, open and close out of control; spleen deficiency, water and dampness can not be transported, muscle atrophy and weakness; coupled with the multifaceted impact of related organ pathology, so prone to disease.  In middle and old age, the functions of the kidneys, spleen and other internal organs are in slow progressive decline, and the bladder is out of function in qi-chemistry, with weakened functions of clearing and secretion; kidney deficiency is not solid; at night, yin energy is strong and yang energy is weak, so bladder failure is especially obvious. The result is frequent urination, increased nocturnal urination, and difficulty in urination, along with the long-term “struggling” urination, resulting in localized gas stagnation and blood stasis in the waterway, and the combination of stasis and dampness, resulting in localized prostate enlargement and even hardening.   The main reason for this is that it is a very good idea to use a Chinese medicine treatment to warm the kidneys and benefit the qi, invigorate the blood and promote diuresis. For BPH with multiple etiologies and symptoms, we should differentiate between individuals, focus on the main factors and take into account the secondary factors in a phased manner, and treat both the symptoms and the root cause with flexible treatment methods, which can achieve a better efficacy-price ratio. According to the different types of evidence, the comprehensive use of tonifying the kidney, warming the yang and tonifying the kidney, benefiting the qi and strengthening the spleen, activating blood circulation and removing blood stasis, clearing heat and dampness and other methods to identify and treat the evidence.  1.Weakness of kidney qi . Frequent urination, weak urination, increased nocturia, decreased sexual function, soreness and weakness of the waist and knees, dizziness, tinnitus, etc. Pale tongue, white coating and weak pulse. It is advisable to tonify the kidneys and benefit the qi or warm the yang to tonify the kidneys, using Jin Kui Kidney Qi Pill or Ji Sheng Kidney Qi Pill plus or minus.  2. Spleen deficiency and Qi trapping. Increased residual urine, impaired urinary emptying of the bladder, chronic urinary retention, therefore frequent or unfavorable urination, abdominal drop, tiredness and weakness, good breath, dullness, etc. The tongue is fat, light and tender, with thin white fur and a sunken pulse. It is advisable to benefit the qi and strengthen the spleen, raise the yang and lift the spleen, and can be used to supplement Zhong Yi Qi Tang with Wu Ling San plus or minus.  3.Stasis blocking Qi deficiency. Long-term breath-holding and straining to urinate, stagnation of Qi and blood stasis at the outlet of the waterway, stasis and obstruction together make the waterway swollen into Y, narrow outlet, obstruction, and even lead to acute urinary retention. The tongue is light and dark with white coating and weak pulse. Treatment is appropriate to benefit the kidneys, tonify the qi and resolve stasis, with internal or local medication.  4. Stasis-dampness and heat. Referring to those with BPH co-infection. Frequent urination, short urination, red heat, urine dripping out, or even urinary closure. Pain in the stem and fullness in the abdomen. The tongue is red, the coating is yellow and greasy, and the pulse is slippery. The treatment is to clear heat and dampness, move blood stasis and promote lymphatic flow, using Bazheng San plus or minus.  In case of serious complications or tumors, a combination of Chinese and Western medicine, including surgery, should be used.  Prevention and care 1. Pay attention not to hold urine and keep the bowels open.  2. Be cautious in living, avoid wind and cold, avoid alcohol, strong tea and less spicy and stimulating food.
