A routine urine test with occult blood (++++) suggests the presence of a hemorrhagic lesion in the urinary tract and requires further examination to determine the location of the bleeding and its cause. First, if, in addition to occult blood (++), there are also positive leukocytes or positive leukocyte esterase, the urinary tract infection is considered to be the cause. Patients are mostly accompanied by symptoms of frequent urination, urinary urgency, and painful urination, and because the local mucosa is bleeding and congested after infection, this will show positive occult blood, and anti-infection treatment is needed. Secondly, if there are stones in the urinary system, such as stones in the ureter and kidney, the stones may cause local mucosal bleeding, so the result is positive for occult blood, and ultrasound examination of the urinary system is needed to confirm the diagnosis of the presence of stones. Thirdly, if the repeated occult blood is positive in elderly patients, it is necessary to consider whether there are tumors in the urinary tract, such as tumors in the bladder, kidney, ureter, etc. It is recommended to perform CT examination of the urinary system to rule out the presence of tumors.
