Treatment and prevention of pain in the upper extremities and shoulders when pulling downward or holding heavy objects in the hands

The most common first symptom of cervical rib is a dull pain in the hand and shoulder that is intermittent. The pain worsens when the upper extremity and shoulder are pulled downward, or when heavy objects are held in the hand. Cervical ribs are a congenital deformity of the rib cage and can be caused by trauma, weight bearing factors on the shoulder, etc. Most cervical ribs are asymptomatic and only show symptoms when the blood vessels and nerves are compressed. The general signs of this disease are that the patient has a full shoulder with many muscles, a shallow supraclavicular fossa, and sometimes a bulging mass or a hypertrophied rhomboid muscle can be palpated. The incidence of cervical ribs is 0.5% to 1%, and it is more common in women, with a male to female ratio of 1:2 to 1:3. The age of first diagnosis is 20 to 40 years. About half of them are bilateral, and unilateral ones are mostly on the left side. Because cervical rib syndrome is mainly a congenital disease, there are no special effective preventive measures. Early diagnosis and treatment is the key to prevention and treatment of this disease, but it should be noted that not all people with cervical rib will have symptoms. Treatment 1.Non-surgical treatment includes massage, physiotherapy, painkillers, strengthening the exercise of scapular muscles, avoiding carrying heavy objects, reducing excessive abduction activities of the affected upper limbs, and taking proper rest. Cervical traction is not effective for this disease. 2.Surgical treatment If non-surgical treatment is ineffective after 3-6 months and the symptoms are more serious, surgical treatment can be considered. 1.Cervical rib resection: it is suitable for type III and type IV cervical ribs which are more completely developed. Generally, the anterior oblique muscle and cervical ribs are cut off via the supraclavicular approach. 2.First rib resection: suitable for type I and II cervical ribs with neurovascular compression due to fiber band. It is usually performed through the axillary approach. Prognosis Cervical rib surgery is effective and has a good prognosis.
