Abstract: Chronic sinusitis is a more common type of sinusitis and is characterized by a long duration of the disease. A 68-year-old male patient was deeply troubled by this disease and reported that he started to feel nasal congestion and loss of smell 2 years ago, and his symptoms were reduced by self-application of cold medication at home. In the past 5 days, the discomfort has increased, so he came to the hospital. After examination, he was diagnosed with chronic sinusitis, and the disease was controlled with medication.
Basic information】Male, 68 years old
Disease Type】Sinusitis (chronic sinusitis)
Hospital】The First Affiliated Hospital of Xi’an Jiaotong University
Time of consultation】August 2014
Treatment plan】Oral medication (cefdinir dispersible tablets, amoxicillin capsule, nasal source and orifice granules) + nasal irrigation (0.9% sodium chloride injection)
Treatment period】7 days of inpatient treatment, 2 weeks after outpatient review
Treatment effect】The symptoms of nasal congestion, loss of smell and clear runny nose disappeared and the condition was stable
I. Initial interview
When I first saw the patient, I was told that the patient had nasal congestion and hyposmia 2 years ago, and occasionally had symptoms of clear runny nose, not accompanied by headache, dizziness, nausea and panic attacks. After listening to the patient’s description, I first conducted a physical examination and saw that the patient’s nasal mucosa was pale and edematous, and there was a small amount of viscous secretion inside, and the perfect sinus CT showed thickened mucous membrane in the nasal cavity.
II. Treatment process
I informed the patient that he was suffering from chronic sinusitis, which was caused by inflammatory lesions in the sinuses and should be treated with medication in a timely manner, and after hearing this, the patient expressed his willingness to actively cooperate with the treatment. I first gave the patient 0.9% sodium chloride injection to flush out the mucus inside the nose, which could relieve the patient’s nasal congestion, and gave him cefdinir dispersible tablets and amoxicillin capsules for anti-infection treatment to slow down the spread of inflammation and achieve the purpose of sterilization. In addition, the patient should be given nasal abnormalities and clearing heat, which can help improve the patient’s nasal discomfort.
Treatment effect
Before treatment, the patient showed symptoms of nasal congestion, decreased sense of smell and clear runny nose, accompanied by headache. After 7 days of treatment, the patient reported that the symptoms of nasal congestion, hyposmia and clear runny nose had subsided and no other discomfort symptoms appeared. The patient was allowed to be discharged after the CT examination of sinuses showed no thickening of the nasal mucosa and the physical findings were normal. 2 weeks later, the patient was in good mental condition, did not complain of discomfort and had no tendency to relapse.
IV. Notes
I was very happy to see that the patient was happily discharged from the hospital. At the time of discharge, I repeatedly advised the patient to rest regularly, to keep his mood relaxed, to do some exercise according to his condition, such as walking, jogging, tai chi, etc., and to consume protein-rich diet, such as eggs, milk, fish, pork, etc., to help maintain the normal functioning of the immune system and assist in the improvement of his condition. In addition, if symptoms such as nasal congestion and runny nose reappear after discharge from the hospital, you should seek medical help in a timely manner and do not use medication at will, so as not to make the condition worse by using wrong medication.
V. Personal insight
Chronic sinusitis is mainly manifested as nasal congestion, loss of smell, runny nose and other symptoms, which are easily confused with cold and flu, so the best time for treatment is often missed. Therefore, once the above-mentioned symptoms appear, you should pay attention to them and consult a doctor in time, not to blindly use drugs at home, so as not to delay the treatment of the disease. Just like the 68-year-old male patient in this case, he initially mistook chronic sinusitis as a cold and did not take targeted measures, resulting in a prolonged illness that seriously affected his work and life, but fortunately, his condition was controlled after treatment.
