This disease is called progressive hemifacial atrophy, also known as Romberg syndrome, was first discussed in detail by Dr. Romberg in 1846, before that Parry had made the earliest report in 1825, so also known as Parry Romberg syndrome. The disease is more common in females than males, and generally begins to develop in late adolescence, starting with skin atrophy on one side of the face (more common on the left side), gradually spreading to subcutaneous fat, fascia, muscle, and zygomatic, temporal, and maxillary and mandibular tissues, etc. The lesions generally do not exceed the midline, with clear boundaries of normal tissue, often appearing in the frontal area or slightly off a demarcation indentation, like a knife cut, called “saber scar”. The lesion develops chronically and progressively, but can stop being stable at any stage. Along with tissue atrophy, skin depigmentation or darkening, hair loss or graying, excessive sweating or sweat closure, and decreased salivary secretion on the affected side may occur. Individual cases also have trigeminal neuralgia, facial sensory disturbances or seizures. The cause of progressive hemifacial atrophy is not genetically predisposed, and the pathogenesis is still unclear, including the scleroderma theory, sympathetic nerve disorder theory, trigeminal interstitial neuritis theory, vasomotor neurotropism theory, and infection theory, but none of them can be completely explained. The treatment of hemifacial atrophy should depend on the degree and extent of the deformity to choose different surgical methods. At present, there is no specific treatment, and plastic repair can only implant filler tissue to obtain a fuller appearance, and strive to be symmetrical with the normal side, to restore the basic normal appearance. The materials used for filling are divided into autologous tissue and scaffold materials, autologous tissue has autologous fat particles free transplantation, dermal fat tissue transplantation, muscle flap transplantation, fascial flap transplantation and so on. The dermal fat flap free graft with anastomosis is the most ideal because the tissue can be fully viable and the graft is more in line with the physiological anatomical characteristics.
