Management of scar adhesions and depressed scarring

Scar is the inevitable product of wound healing. If there is also skin, subcutaneous tissue or even deep tissue injury or infection, the wound will be healed by scar adhesions, forming a depressed deformity with a surface significantly lower than the surrounding normal skin surface, affecting the appearance. Treatment of scar adhesions (a) Needle puncture method 1. Method 1 After local anesthesia, a 16-gauge needle is used to puncture and release the scar at the junction of the scar and normal tissue in a fan shape. 2. Method 2 A common 10mL syringe attached to a needle is used to fan puncture and loosen the scar at the junction of the scar and normal tissue during the implementation of fat graft volume assessment for epinephrine saline injection, or during the injection of local anesthetic. (ii) Thread cutting method 3-0 or 4-0 silk thread attached to a common household suture needle or Keith needle is passed subliminally along the perimeter of the adhesions and at a distance of 1 cm from the junction of scar and normal tissue, with the same point of entry as the point of exit, and the rough surface of the suture is used to cut the adhesions through the ends of the movable thread to loosen them. (iii) Toledo fork separation method With a variety of different sizes, the application makes an incision in the skin according to the width of the fork and then separates the adhesions. coleman makes a lateral hole behind the bifurcation for more convenient operation. (iv) Small needle knife method After local anesthesia, a small needle knife is used to puncture and release the scar at the junction with normal tissue in a fan shape. (v) Direct release method For those who have an adnexal scar, the adnexal scar is cut and then bluntly separated between the scar and normal tissue with mosquito-jaw forceps to form a cavity.
