Self-care after artificial vascular endovascular fistula for hemodialysis

  1.Swelling of the affected limb will inevitably occur after surgery, and the swelling time will generally last 1-3 months, so the limb on the operated side should be elevated to promote venous blood flow back to reduce the degree of swelling until it subsides.  2.For 5-7 days after surgery, keep the surgical side of the limb changed every day and keep it clean and dry, do not remove the dressing at will to prevent wound infection; if you find that the wound is bleeding more and the pain is unbearable, please inform the medical staff immediately for proper treatment.  3.In the early postoperative period, try to wear loose cuffed clothes; when local redness and swelling are obvious, use 50% ethanol wet compress.  4.The limb on the operated side can do fist clenching action or wrist movement properly 3-5 days after surgery to promote blood flow and prevent thrombosis. Take anticoagulants as prescribed by the doctor and follow up the coagulation image and blood routine regularly.  5.Check the functional status of the artificial blood vessel at regular intervals (early in the morning, before going to bed and after discomfort), touch the side of the vein with the non-operative side hand, if tremor is detected or vascular murmur is heard, it indicates that the artificial fistula is open; if there is no tremor, no pulsation, the vascular murmur is reduced or disappears, or there is radiating pulsation, the medical staff should be informed immediately to further determine whether the artificial blood vessel is occluded and to win rescue time.  6, the operated limb should not be weight-bearing, do not wear watches or jewelry and other items, do not wear clothes with narrow or tight cuffs, and pay attention not to be pressurized during sleep; prohibit therapeutic activities such as blood drawing, infusion, transfusion, blood pressure measurement on the endovascular side of the limb.  7, effective control of blood pressure during and between dialysis treatment, to avoid excessive weight gain during dialysis, to prevent hypotension and thus cause thrombosis of the artificial vascular endovascular fistula.  8.After dialysis, please use your thumb, index finger or middle finger to compress the puncture point with the strength that can stop the bleeding and find the pulsation of the blood vessel. This method is the least traumatic to the artificial blood vessels and has good hemostatic effect. Generally, the bleeding can be stopped in about 15 minutes. If the time is too long, inform the medical personnel and check whether there is outflow tract stenosis.  9.When local hematoma appears in the artificial blood vessel, acupressure should be given immediately, and when subcutaneous hematoma appears, ice can be applied, and massage with Xitropin. 24 hours later, hot compress will be applied again to promote subcutaneous hematoma to subside, and do not puncture dialysis at the hematoma for the time being.  10. Do not wash the puncture site on the day after dialysis, and cover it with a band-aid for 6-8 hours to prevent infection.
