Postherpetic neuralgia (PHN) is a painful condition that persists even after acute herpes zoster has been clinically cured, and is one of the most persistent pains in the middle-aged and elderly population. Patients suffer from prolonged pain, and in some cases, the pain is unbearable. The pathological changes are not yet fully understood. Some data show that the nervous system of patients with postherpetic neuralgia is extensively and severely damaged by the virus, the posterior root ganglion is dehydrated and degenerated, the number of ganglion and peripheral nerve cells is significantly reduced, especially the axons of myelinated thick nerve fibers are reduced and fibrotic, and chronic inflammatory cell infiltration can be found in the posterior root ganglion. Narcotic analgesics Narcotic analgesics are not as effective as in other areas of pain treatment for patients with postherpetic neuralgia, but narcotic analgesics are still used for pain relief at home and abroad. 2, antidepressants antidepressants used in the postherpetic neuralgia patients with a certain effect of adjuvant analgesic treatment, commonly used clinically are amitriptyline, doxepin, sellett, etc., can be used as a routine, the use of the process should pay attention to the small dose and gradually increase the dose to prevent side effects. 3, antiepileptic drugs antiepileptic drugs alone is not obvious, combined with antidepressants can improve the efficacy, clinically commonly used are carbamazepine and phenytoin sodium, the use of the process should pay attention to liver and kidney function. 4.Local medication For patients with obvious local skin irritation symptoms, i.e. irritation and tenderness type postherpetic neuralgia, the use of lidocaine, aspirin, capsaicin and other emulsions with analgesic effect can achieve certain therapeutic effects. 5.Immunomodulators Although we do not know the exact connection between immune factors for the occurrence and prognosis of postherpetic neuralgia, it is recognized that the occurrence of acute herpes zoster is closely related to the decrease of the body’s immunity, so immunomodulatory treatment should be one of the directions, early clinical use of polyinosinic acid and nucleotides. Acupuncture is a traditional Chinese medicine treatment method in China. The analgesic effect of this method is certain, and its basic principle is based on the principle of acupuncture for pain relief. According to our preliminary clinical experience, regional nerve or nerve root injection is currently the most effective method to relieve severe pain in patients with postherpetic neuralgia, especially for the duration of the disease.
