How is follicular lymphoma treated?

  Malignant lymphomas can be divided into two major groups, Hodgkin’s lymphoma and non-Hodgkin’s lymphoma, according to the pathological typology. The incidence of the latter is significantly higher than that of the former, accounting for more than 90%. Therefore, when talking about lymphoma in general, it means non-Hodgkin’s lymphoma. According to the tumor growth rate, non-Hodgkin’s lymphoma can be divided into three types, namely inert lymphoma with slow growth, aggressive lymphoma with fast growth and highly aggressive lymphoma with very rapid growth. Follicular lymphoma (FL) is an inert lymphoma. Conventional treatment is not considered curative; the median survival of FL patients is 8-10 years; most patients are elderly and often have comorbidities that affect treatment options; “watch” and “wait” may be more appropriate for patients without indications for treatment “Treatment”; no improvement in overall survival with standard chemotherapy regimens; progressively shorter effective (remission) period after each treatment; recommended for enrollment in clinical trials if eligible. According to the Follicular Lymphoma International Prognostic Index (FLIPI) score: whether age >60 years, stage III/IV, hemoglobin <120 g/L, >4 lymph node area invasion and elevated lactate dehydrogenase (LDH) levels, three groups were classified as low, intermediate and high risk, and their ten-year survival rates were 71%, 51% and 36%, respectively.  Therefore, the treatment selection of FL is now more individualized. Both the patient’s age, symptoms, immediate and long-term treatment goals, comorbidities, and affordability should be considered, as well as the stage of the disease, FLIPI index, the presence of pathological type transformation, lesion invasion site, previous treatment regimen, and the interval between previous treatments to date. In order to improve the quality of life of patients, reduce the related symptoms and prolong the remission time. Regardless of the means taken, TCM treatment can be used throughout.
