The so-called due date is just an approximate date. A child delivered 3 weeks before and 2 weeks after the due date is called a full-term baby, meaning that the fetus is all mature. There are several signs of a mature fetus: the fetus has grown to more than 2500g in weight; there is thick hair on the head; the finger (toe) nails have grown past the ends of the fingers and the palms of the hands (feet) have clear palm lines; the folds on the skin have unfolded and the sweat hair on the body has decreased; the testicles of the male baby have descended into the scrotum and the labia majora of the female baby have bulged. Thus we determine whether the fetus is full term from the time and the appearance of the fetus. However, few pregnant women give birth on their due date because the due date is only an approximate date. Therefore, when the due date passes, many people are anxious and think that something is wrong. After this time, the uterus will not continue to grow, the placenta will gradually tend to age, the amniotic membrane will gradually weaken, and the amniotic fluid will gradually decrease, but the fetus will continue to grow, so a time limit has been set in medical science, and 2 weeks after the due date is considered an overdue pregnancy. After the 21st century, people have better living conditions, the pampering of only children, the development of science and technology, the increase of infertility, so now society takes extra care of pregnant women, families take extra care of pregnant women, pregnant women themselves are under great psychological pressure, worried about the end of pregnancy. Therefore, pregnant women are often very little activity after pregnancy, not to mention exercise. The exercise of the pregnant woman herself increases the elasticity of the muscles and the toughness of the joints and ligaments. When the fetus is mature, the fetus will secrete a hormone into the mother’s bloodstream, prompting the mother’s pituitary gland to secrete oxytocin and initiate contractions and labor, which is what we often call “ripe for the picking”. After labor, the fetus is actively looking for a suitable meridian in the mother’s pelvis to rush outward; the mother is calm after labor, and as labor progresses, the fetus gradually descends under the push of bouts of uterine polarity and symmetric contractions and abdominal muscle contractions, and when the fetal head reaches the pelvic floor, the contractions of the pelvic floor muscles push the fetus to be delivered smoothly. When the maternal activity is reduced, the fetus lacks the power of passive movement given by the mother, so the fetus has poor initiative and cannot actively secrete hormones to the mother. The lack of movement of the mother and the lack of elasticity of the muscles make the small amount of oxytocin insufficient to initiate contractions, so that labor is delayed and the pregnancy is overdue. When there is no sign of labor after the due date, the following things should be done: 1. Check the due date: The first thing to do is to determine if the due date is reliable, as some people have irregular periods, some cannot remember the time of their last period, etc. It is important to check the due date again with the help of the doctor. The method of checking the due date: the exact date of the last menstrual period; when and by what method you knew you were pregnant after the last menstrual period; the earliest time of early pregnancy reaction (nausea, vomiting, drowsiness, dizziness and headache, breast swelling, emotional vulnerability, etc.); the week of pregnancy indicated by the first ultrasound; the time of the first fetal movement; based on the above examination and self-perception, you can usually accurately calculate the due date the time of the first ultrasound, and determine whether the pregnancy is overdue according to this time. Do not determine the expiration date according to the ultrasound at the end of pregnancy, because the birth weight of the fetus varies in size, there are 8 pounds, there are 5 pounds, 8 pounds is considered full-term, 5 pounds is considered not yet time, it will be a mistake. 2, self-monitoring: more than the expected date of delivery pregnant women to self-monitoring, the best way is to observe fetal movement. The so-called observation of fetal movement is to often pay attention to whether the fetus is active, whether the activity is the same as usual. If the fetal movement is reduced or apparently not moving, you should immediately go to the hospital to see a doctor. Since there is no pattern to the fetal movement, don’t scare yourself. The fetal sleep-wake cycle is forty minutes, and the fetus usually doesn’t move for about an hour, and then it will appear after a while. If it does not move, you can also touch the abdomen and pat it gently; or clap your hands and observe again. 3, regular perinatal health care: the rule of our prenatal checkups during pregnancy is about once a month before 27 weeks, mainly to screen for fetal abnormalities. 27-36 weeks is generally 2-3 weeks, mainly to understand the health status of the fetus and the mother, such as whether the mother has combined gestational diabetes, gestational hypertension, diet and nutrition, psychological and emotional, etc.; for the fetus, mainly to understand whether the size of the fetus matches the gestational week, how much amniotic fluid is present, and how much fetal fluid is present. From 36 to 40 weeks, the fetus will be checked once a week to understand the development of the fetus in the uterus, whether the fetal position is normal, to determine the safest way to deliver the baby and the mother, and to provide timely guidance on the delivery method. After the due date, you should come to the hospital every three days for a checkup. During the checkup, the doctor should understand the size of the fetus, the position of the descending fetal head, the amount of amniotic fluid and the fetal movement through four-step palpation. When it is found that the amniotic fluid decreases and the uterus is tightly wrapped around the fetal body, it is necessary to understand how much amniotic fluid through ultrasound examination, which generally indicates that the amniotic fluid index ≤ 5cm is indicative of too little amniotic fluid, and if the amniotic fluid index ≤ 8cm indicates little amniotic fluid, it should be actively dealt with. If the fetal head is delayed to enter the pelvis, and the doctor presses down the bottom of the uterus, and the fetal head is higher than the pubic arch, it means that the fetal head is in a bad position, and there is fetal head riding across, meaning that the fetal head cannot enter the pelvis, then we can’t wait any longer and should deal with it actively. Generally, beyond the expected date of delivery, the fetus grows gradually and the skull becomes hard, making it difficult to shape the skull during delivery and causing difficulties in delivery. If the fetal heart monitor pattern is abnormal, such as fetal heart baseline “small variation missing, flat baseline, sine type, no change in baseline during contraction and fetal movement, fetal tachycardia, fetal bradycardia, etc., it indicates that the fetus is in danger in the uterus. It is important to deal with it actively and not to wait any longer. After the first trimester, the doctor will request an ultrasound to determine the size of the fetus, the umbilical hematocrit and the fetal biophysical score to assess the risk of the fetus in utero. When the umbilical hematocrit is ≥3, the fetus is hypoxic in utero. The fetal biophysical score has five items, each with 2 points: fetal heartbeat, fetal movement, amniotic fluid, respiration and muscle tone. 4. Hospitalization at the right time: Generally, when the pregnancy is 1 week past the due date, that is, 41 weeks, the doctor will admit the fetus to the hospital and deal with it according to the condition of the pregnant woman’s cervix. If the fetus has intrauterine distress, a huge baby, or a positive fetal head across the pubic sign, the doctor will recommend a cesarean section to end the delivery to ensure the health of the mother and baby. If the fetus is normal and simply overdue, the doctor will induce labor depending on the condition of the cervix to promote maturation of the mother’s cervix and cause spontaneous contractions and prodromal labor. The process of delivery after this type of prodromal labor is no different from natural prodromal labor, and the process of labor progression is the same, except that the doctor will pay more attention to the observation of the fetus tolerating contractions in the uterus, so as to detect fetal abnormalities and deal with them in time. As the fetus grows, the mother’s placenta weakens and the fetus consumes its own fat, so the term baby is born with many head lines, like a little old man, and its immunity and resistance are not as strong as a full-term baby. Therefore, in order to prevent overdue pregnancy, pregnant women must live a normal life, pregnancy is not a disease, is a normal physiological process, positive attitude, not petulant, peace of mind, do not be afraid of the wolf before and the tiger after, to be active, participate in labor, moderate exercise, to avoid the occurrence of overdue pregnancy. When close to the due date there is no feeling, pregnant women can massage their own nipples, this massage nipples will prompt the pituitary gland to secrete oxytocin to cause uterine contractions, generally 20 minutes each in the morning and afternoon, through the nipple stimulation, to promote early labor.
