I. Concept: Luteal insufficiency means that the corpus luteum is not fully formed after ovulation, resulting in insufficient progesterone secretion, so that the endometrium is not converted in time, which is not conducive to the fertilized egg’s implantation. Therefore, it often leads to infertility or habitual abortion. There is no clear record of this disease in Chinese medical literature, but modern research believes that kidney deficiency is an important factor in this disease, and clinical deficiency of kidney Yang is especially common. Etiology: Modern medicine believes that the cause of this disease is not too clear at present. The maintenance of normal luteal function depends on the perfect functioning of the thalamus-pituitary-ovarian gonadal axis, so most scholars believe that the cause of luteal insufficiency may be related to insufficient luteinizing hormone (LH) and follicle stimulating hormone (FSH) secreted by the pituitary gland, too much or too little prolactin (PRL) secreted by the pituitary gland, and immature follicles themselves. The luteinizing gland itself does not synthesize enough progesterone or the ratio between progesterone and estrogen is not coordinated. In Chinese medicine, it is believed that congenital deficiencies in endowment, or acquired wind, cold, damp, and cold evils, multiple childbirths, prolonged illness, overwork, and emotional disorders can easily lead to deficiency of kidney qi and kidney yang. In addition to the deficiencies of the spleen and kidney, the deficiency of kidney qi and kidney yang, the deficiency of kidney yang can also be transformed from the deficiency of kidney yin, the deficiency of sebum and yin, the follicle is not very mature, clinically it can be seen that small follicles ovulate and the basal body temperature curve does not rise normally after ovulation. It can be seen that yin deficiency is transformed from yin to yang necessarily deficient, therefore resulting in yang deficiency, yin in yang deficiency, water fire weakness, clinically manifested as luteal insufficiency, in fact, is the pre-ovarian follicle dysplasia. Clinical manifestations: Luteal insufficiency can easily lead to follicular dysplasia, incomplete luteal production and imbalance in the ratio of estrogen and progesterone secreted by the corpus luteum itself. There are many infertility cases caused by luteal insufficiency, accounting for about 3-10% of all infertility cases. The main manifestations of luteal insufficiency are shortened menstrual cycle, frequent menstruation, prolonged periods, infertility or miscarriage, and miscarriage mainly occurs in early pregnancy. Patients can usually appear back pain, fatigue, fear of cold, cold hands and feet, a little bleeding before menstruation, breast swelling and other uncomfortable symptoms. Fourth, Chinese medicine treatment: treatment should start from the whole, filling the kidney essence and regulating the menstrual blood. The main method is to replenish the kidney energy, benefit the essence and blood, cultivate the vital essence and regulate the flushing. According to the different stages of the menstrual cycle, the balance of yin and yang in the kidney should be regulated in stages, and the endocrine disorder should be adjusted by adjusting the function of the body and improving the body’s own regulation ability. We commonly use Chinese herbal sequential method to treat luteal insufficiency infertility in clinical practice. 1. Follicular phase: The late menstrual phase is the follicular development phase, which is the first key period of sequential therapy, and is now a Yin phase according to the Yin-Yang dialectic, with the basal body temperature in the low temperature phase. Kidney yin is the material basis of reproductive function. During this stage, we should focus on nourishing kidney yin, tonifying the spleen and kidney, nourishing yin and blood, so that the true yin and yang in the kidney can be enriched to promote the full development and maturation of the follicle and the arrival of the “true opportunity”. In other words, it helps the endometrium to repair and proliferate and the follicles to develop. This formula is often used clinically with the addition and subtraction of Gui Ren Wan and Er Zhi Wan. It is important to nourish Yin and Blood during this period, but it is even more important to nourish essence and help Yang. On top of the drugs that nourish Yin and Blood, add a small amount of gentle Yang-aiding drugs to help the growth of Yin essence (follicles), such as Serpentine, Cuscuta and Zi He Che. 2, premenstrual period: that is, the luteal phase, after ovulation, the basal body temperature enters the high-temperature phase, during this period Yang grows and Yin disappears, the Qi and blood of the flush is full, Yang gradually grows, liver qi is easy to disturb along the meridians, often accompanied by breast swelling and pain, chest tightness and irritability and other symptoms of liver depression. Luteal insufficiency, mainly related to kidney yang deficiency. The Yang grows to heavy during the premenstrual period, and the Yang growth starts under the transformation of heavy Yin, so the Yang grows to heavy and must have the foundation of real Yin. The deficiency of kidney yang is actually a deficiency of yang-water, which to a certain extent will also affect the deficiency of qi and blood, thus also affecting the transportation function of the spleen and stomach, resulting in deficiency of both spleen and kidney. Therefore, during this period, the main treatment principle is to warm the kidneys, strengthen the spleen and promote yang, and at the same time to dredge the liver and regulate qi to promote the development of the corpus luteum to facilitate the development of the pregnant egg. The medicine uses deer horn tablets, cuscuta, cistanches, bacopa monniera, Epimedium, Fructus Lycii, Atractylodes, Poria and Chen Pi. However, attention must be paid to replenishing fire in water, seeking yang in yin, and adding products that nourish yin while warming kidney yang. Clinical treatment also needs to be based on the patient’s different physical constitution and intermingled evidence, combined with the four diagnostic conditions, flexible diagnosis and treatment, treatment of this disease must be taken regularly according to the menstrual cycle, starting from the late menstrual period, adhere to 3 menstrual cycles.
