Proper treatment of annoying breast milk (physiological) diarrhea

Breastfed babies often have more frequent stools, and young parents are more worried about their baby’s nutrition, and bothered by diaper changes and red bottoms. How to correctly understand and treat diarrhea in breastfed babies? Breastfeeding (physiological) diarrhea characteristics: mainly occurs in breastfed babies within 6 months, especially newborn babies, newborns in the first few days of breastfeeding, the number of stools is more, the stool is thinner, about 2 weeks after the physiological diarrhea baby stools continue to thin, 4 to 6 times a day, more than 10 times, and even after the full moon, the number of stools and stool nature is still not seen improvement. The stool is thin and watery, with milk flaps or with a little transparent mucus, often after feeding. There was no fever, no pain, no crying, no abnormalities in infection, good mental status, no change in appetite, and growth and development were not affected. There is no dehydration, and stool tests are not abnormal. Even though the duration of diarrhea is long, the baby’s weight is still increasing (300g or less every 10 days). Some babies are a bit fat and often have milk ringworm on the face, behind the ears or in the hairline. Diarrhea can be cured after 4 to 6 months, with the increase of complementary foods, digestive function tends to be perfected. Causes 1. Temporary intolerance to lactose: Many newborns and preterm infants are temporarily intolerant to lactose during the neonatal period due to immature development of the intestinal mucosa and temporarily low lactase activity, and defecate more often. Subsequently, the enzyme activity is induced by the increase of lactose content in breast milk, and the number of stools decreases when the activity is normal. As the amount of enzyme deficiency and the degree of activity varies from person to person, the severity of symptoms varies; 2. High prostaglandin content in breast milk, the substance promotes the smooth muscle movement of the small intestine, increasing the secretion of water and electrolytes and producing loose stools; 3. Excessive lactose: sufficient lactose, sufficient milk to drink only (the truth may be as simple as that). Colostrum lactose content is low, so the first 2 weeks after birth stool frequency is not much. Note: congenital lactase deficiency is not physiological diarrhea anymore. It is caused by a congenital deficiency or lack of activity of lactase, which is rare and belongs to autosomal recessive inheritance. It is rare and is autosomal recessive. It means that the lactase activity of the body is low or deficient from the time the baby is born. Judgment method: no matter drinking breast milk or cow milk, it can lead to obvious diarrhea, stop feeding breast milk or replace with rice soup type food feeding, diarrhea disappears, that is, congenital lactose deficiency or low. The symptoms of diarrhea mainly appear 1 to 2 hours after breastfeeding in newborns, accompanied by abdominal distension, hyperactive bowel sounds, spasmodic diarrhea, and in severe cases, vomiting, water loss, and acidosis. The stool is watery, foamy, acidic and contains lactose. Signs: no weight gain, malnutrition, wasting, anemia, abdominal bulging, etc. What to do with breastfeeding (physiological) diarrhea Generally, mild physiological diarrhea does not require treatment and can be continued with breastfeeding. In this way, the infant’s organism will gradually adapt to the prostaglandins in breast milk, lactase will also gradually develop and mature, the enzyme activity will increase, and gradually be able to break down, digest and absorb lactose, and gradually improve with the addition of complementary foods. Changing to formula feeding can be effective, but it is not recommended to change to formula feeding for the sake of stool formation. “Physiological diarrhea” does not hinder the normal development of the baby, and giving up breast milk and switching to formula is a choking approach. As long as the diarrhea does not affect the baby’s development and health, the mother should not worry too much. Perhaps lactase can be added. Also note that children should not cover their nipples, after each side of the breast is basically empty, if you continue to suck, the amount of fat in the milk will increase, which can easily cause lipid indigestion, advocate to suck, do not suck to stop. Each time about 10 minutes, the full breast can be basically sucked empty. Note: If the bowel movement is more than 10-20 times a day, “egg-flake” watery, the baby shows symptoms of dehydration such as crying, little or no urine, and mental depression, he/she needs to be sent to the hospital for treatment in time. The main points of the care of small buttocks when diarrhea: 1, usually as soon as the poop to immediately wash with water, do not dry, gently dip dry with a soft cloth, if possible with a hair dryer micro-warm air dry, and then coated with something to protect the small buttocks. You can use vegetable oils such as sesame oil, tea oil, olive oil, etc.; Bowing ointment, tannic acid ointment, etc.; sudocrem buttock cream from the UK, Johnson & Johnson baby buttock cream, and any buttock cream containing zinc. Usually the higher the zinc content, the better the effect; 2. Before applying the cream buttock protector, make sure the little buttocks is dry because if it is not, the cream will wrap the moisture on the skin and will make the red buttocks and diaper rash more serious. However, do not use talcum powder, talcum powder running to the genital area is harmful to the baby, and talcum powder will make diaper rash worse; 3. If the buttocks ulcerated, there is an effective physical therapy, which is a baking lamp, the principle of which is to promote the drying of the wet parts of the small buttocks through the heat of the light source, so that the skin damage, rashes, red spots heal. Use a 25W incandescent bulb in a desk lamp, let the baby pout on his back, bake at a distance of 15-20cm to the buttocks, 10-15 minutes each time, 3-4 times a day; 4. In serious cases, you need to go to the hospital to prescribe ointment, hormonal ointment for short-term use is also possible, do not use drugs indiscriminately by yourself; 5. Usually keep the small buttocks dry as much as possible at home, such as having as much time as possible every day without Wear a wet diaper and let the skin come in contact with the air. You can also let your little buttocks sunbathe every day in the morning and afternoon when the sun is not too fierce, and the ultraviolet rays help to heal and recover; if your baby usually farts a lot and cries, it is flatulence. After each feeding to the baby carefully pat clusters, you can also give the baby 3-4 times a day abdominal massage, clockwise palm lightly massage the abdomen 30 times.
