OBJECTIVE: To introduce a laparoscopic cholecystectomy procedure in which no surgical scar is visible in the abdomen. METHODS: A 10-mm poke hole was made in the umbilicus for the main operation and removal of the gallbladder; a 5-mm poke hole was made at each end of the upper edge of the scrotum, one for access of the 5-mm laparoscope and the other for access of the grasper. The main operation was performed with a 5mm ultrasonic knife to dissociate the gallbladder duct and gallbladder vessels, followed by separation of the gallbladder bed with an electrocoagulation hook and then ligation of the gallbladder duct with absorbable trap wire and ultrasonic dissociation. The excised gallbladder was removed through a poked hole in the umbilicus. Finally, the umbilicus was closed with absorbable thread and the poked hole at the pubic hair was covered with adhesive tape pulled together. Results: 25 female patients with benign gallbladder lesions, 19-35 years old. The operation was successful in all of them, and the operation time was 25 min (20 min-35 min). None of them had drainage placed, and no postoperative complications such as bleeding or bile leakage occurred. Postoperative hospitalization was 1-3 days. At the follow-up of 1-3 months, the punched holes were well healed and no surgical scar could be seen at the umbilicus and pubic hair. The patients were satisfied with the results of the procedure. CONCLUSION: Laparoscopic cholecystectomy with the poke hole located at the umbilicus and the upper edge of the hidden hair can achieve the purpose of hiding the surgical scar.
