The 2006 GINA guidelines suggest that discontinuation of asthma control medications may be considered when patients are able to maintain asthma control at the lowest dose of control medication if they are no longer experiencing recurrent asthma symptoms for up to 1 year (level of evidence D). Asthma expert’s concept and practice: the essence of asthma is inflammation, and the fundamental aim of asthma treatment should be to eliminate airway inflammation; control as defined by current guidelines can only be clinical control. Therefore, monitoring and assessing airway inflammation should be an important part of asthma management. Before considering discontinuation of medication, we (asthma specialists) would choose airway responsiveness measurement to assess inflammation control to assist in determining whether discontinuation is appropriate, which is more reasonable and scientific.
