It is very common to see elderly patients over 60 years of age with high blood pressure in the recumbent position when lying down, high blood pressure in the sitting position when sitting, but not as high as when lying down, but an excessive drop of more than 20 mmHg in the standing position after standing up, which makes clinicians feel very headache. Today, a patient’s blood pressure was 178/90 mmHg in the prone position and 163/86 mmHg in the sitting position. Such a blood pressure makes it difficult for the physician to treat. If the recumbent blood pressure is lowered to normal, the standing blood pressure will be too low, causing dizziness and weakness due to insufficient blood supply to the brain. And prolonged recumbent blood pressure is an important cause of heart disease. Currently there are no effective medications for such a condition in Western medicine. In this case, postural hypotension occurs in patients with hypertension. There are primary and secondary forms of postural hypotension. Primary hypotension is caused by a neurological disorder in the vascular regulating center of the brainstem, resulting in uncontrolled regulation of blood pressure, and when standing, blood sinks downward due to gravity, resulting in ischemia in the upper part of the body and low blood pressure. There is also a secondary condition, which is caused by the failure of the body’s pressure chemoreceptors in the neck due to atherosclerosis in the elderly, resulting in the loss of feedback regulation of lower blood pressure, leading to low blood pressure in the standing position. Currently, Western medicine uses compression stockings and reduced self-supporting activities for this disease. There is no effective western drug treatment. Chinese medicine can treat this disease, and Chinese medicine is clinically observed to have both hypotensive and elevating effects, so it can only be treated by Chinese medicine. We have achieved some efficacy in the clinical use of herbal treatment. The basic principle of Chinese medicine for such blood pressure is to nourish the kidneys and calm the liver, to relieve the fire and submerge the Yang, to benefit the Qi and invigorate the blood, and to resolve phlegm and dampness. This can restore the body’s regulation of blood pressure. Some elderly people in their eighties have a blood pressure of 170/100mmHg in the recumbent position and 80/50mmHg in the standing position, manifesting as dizziness, unstable walking, red face and weakness after activity, and lower blood pressure. The patient’s dizziness was relieved after the addition of Yang and Wu Tang, but the redness did not decrease after activity, and the blood pressure still fluctuated greatly. The patient was considered to have congenital deficiency of kidney qi at an advanced age, manifesting as a deficiency of both qi and yin, and a deficiency of yang. The patient’s lower extremities are cold in winter and he is afraid of cold. After six months of treatment with tonics, the patient’s blood pressure was stabilized at 140/80mmHg in all positions after one year of treatment with traditional Chinese medicine. The dizziness disappeared and he moved freely. Recently, I treated another case in the clinic where the blood pressure was 160/90mmHg in the seating and lying position and 95/55mmHg in the standing position. This patient had a spleen-yang deficiency and liver-yang hyperactivity, and after a week of using Tianma and Hooked Vine Drink with Wu Ling San and Huang Qi, the blood pressure in the seating position dropped to 125/80mmHg while the blood pressure in the standing position did not drop, but remained 95/55mmHg. The next step is to focus on raising the standing blood pressure, such patients are mainly treated with Chinese herbal medicine. After one year of treatment, the patient’s blood pressure was about 135/80mmHg in either the prone or standing position. Now he has stopped using antihypertensive drugs for several months and his blood pressure is normal.
