A few questions about rhinoplasty

The nose is located in the middle of the face and plays an important role in both front and side views. A beautiful nose can greatly improve the overall beauty of the face. The Oriental race has a flat face and lacks a sense of contour. Many people hope to enhance their facial dimension through minor surgery, and rhinoplasty has naturally become the first choice, and cosmetic rhinoplasty has become one of the most popular plastic surgery procedures in the East. At present, various media have introduced information about rhinoplasty. Most of the candidates have also done their homework before visiting the clinic. However, they still ask two eternal questions: what kind of nose is the most standard nose? What kind of surgery is the best rhinoplasty? To further enhance the understanding of rhinoplasty surgery, we interviewed Professor Wei Min from the Department of Plastic Surgery of the Ninth People’s Hospital of Shanghai Jiaotong University School of Medicine to address these two questions. Professor Wei Min is well known for his work in rhinoplasty and facial contouring, so let’s talk about the vital nose with Professor Wei on your behalf. Q: Professor Wei, first of all, thank you for taking the time out of your busy schedule to answer our questions. Our first question is what kind of nose do you think is the most iconic nose? A: Oh, you guys are too kind. I am a doctor and I am supposed to answer the questions of my clients in my field, and it is not a small question, it is a question that almost all clients ask. First of all, let me clarify my opinion that I think this question is a “pseudo-proposition”, that is, there is no accurate answer that can score one hundred percent. Nowadays, many media reports will talk about “the most beautiful nose”, “the most standard nose”, “the sexiest nose” and so on, and even give a picture with various angles, lengths and curves. It even gives a picture with various angles, lengths and curvatures, which looks very advanced and very cool. In fact, in academic terms, there is an accurate description of the standard nose earlier, but we have to realize two points: 1, rhinoplasty is developed from Europe and the United States, so the anatomy and aesthetics of the nose is inevitably extended to the European and American ideas or thoughts, and our Oriental and European and American nasal anatomical features are different, aesthetic differences, so I do not think that the most standard number with the The nose is the most standard nose; 2, academic things, is a combination of various objective factors, its conclusions are relatively objective. People, on the other hand, are much more complicated. It can be said that there are as many faces and as many types of nose shapes as there are people in this world. Can you say with certainty that the nose shape with standard numbers will look good on all kinds of faces? So, my point is: we definitely have an academic point of view, and within a certain range, we usually move closer to that standard nose shape, not absolutely that it has to be that way, and we can’t use it as a fixed mold to apply it all the time. For each candidate, it is more reasonable to correct the regrets of their nose itself, and at the same time to perform nose reshaping surgery for their face shape, so that their nose shape becomes good-looking and integrates with their face, and at the same time to meet their age, personality, occupation and hobbies. Those who are a little older are not suitable for a nose that is too straight and arrogant, while those who are independent and have a strong sense of facial contour are suitable; a round face can have a cute nose, while a particularly prominent tip or parallel nostrils do not go well with it, but are suitable for people who need to take pictures and photography, etc. Q: Well, then Professor Wei, according to your point of view, our second question also comes: which rhinoplasty can achieve such an effect, that is, it looks good and matches my face shape perfectly; it is effective and safe at the same time? A: This is the case, rhinoplasty surgery classification is more, generally according to the nature of the nose can be broadly distinguished into congenital deformity class, trauma repair class, cosmetic class three blocks, of course, all kinds down to subdivide into N kinds. Today, we will talk about cosmetic surgery alone, which is purely aimed at pursuing the perfect image and is the icing on the cake, with great difficulty and low satisfaction. For those who are only slightly sunken at the root of the nose, they can choose minimally invasive injection rhinoplasty, or those who only have a nose bridge that is not high enough, they can choose implants for simple rhinoplasty. Most of the candidates, however, are not lucky enough to have only a little regret in their nasal shape, but the shape of several subunits of the nose is not ideal, and there is a lack of coordination between the subunits, and the whole nose does not match the shape of the face. At this time, simply raising the bridge of the nose is not ideal and sometimes even counterproductive. For example, some people have low nasal bridge, inconspicuous nasal tip point, over-arched nose, obvious nostril exposure or bad shape, just stuffing a prosthesis does not solve the problem of nasal tip, nose and nostril, more likely: the surgeon only focuses on shaping the prominent nasal tip point when sculpting the prosthesis, as a result, the nose of the candidate is more arrogant, nostril exposure is more obvious, and the nasal tip point is also unnatural. Because just simply entering along the incision for loosening cannot be very effective in performing nasal lengthening, instead, a variety of problems will occur due to tension and other factors. Therefore, we have to observe the morphological characteristics of the candidate comprehensively, lock the nasal shape according to the facial profile, then interpret its nasal shape in detail, find out the deficiencies of each subunit and design the surgical plan. The principle is to unite the nasal bridge, nasal tip point, nasal head, nostrils and nasal column, and also to unite them with the face shape, and to combine several operations in one operation to shape the nasal and nasal-facial harmony, that is to take joint surgery to reshape the nasal shape. In today’s widespread rhinoplasty, we emphasize more on the importance of combined surgical nasal reshaping. Q: Professor Wei, can you briefly introduce this combined surgical approach? A: Yes. We just mentioned why we take combined surgery and its principles. It roughly consists of the following steps: 1. Incision: The incision is made at the low end of the nasal column, and if the nose is enlarged, flared, or sagging, the incision is also made on both sides of the nasal cheek groove. Some people will worry about the incision scars, which is understandable. However, all surgeries have certain incision scars, and high-end surgery is able to minimize and hide the scars. Good technique, natural depression and pigmentation can make the scars almost “disappear”. 2.First of all, reduce the amount of tissue of the enlarged nose. If you simply treat an enlarged nose, then a combination of several methods can effectively remove the amount of tissue. But the question is: does the nose look better? Not to mention problems such as nasal dykes and nostril deformation. Instead, when we use a combined procedure, we can use only one type of excision. The excess amount of soft tissue is removed along the nasal cheek groove. Will this be enough? Probably not; not more than one type of excision? No; why? We will talk about that later. 3. Loosening the skin to lengthen or reduce the tension is not as good as directly “adding” an extra piece of skin, and making a small incision at the base of the nasal column is for this purpose. Separate the flap with minimally invasive principles and untie the nose. Treat the middle foot of the large wing cartilage to improve the large nasal head, implant the sculpted and specially treated material to further improve the large nasal head and shape the beautiful nasal tip, and suture it. 4.For the material, generally we prefer tumescent PTFE as the implant material, it has several advantages, such as natural, its own tissue recognition it and can grow into it, rejection rate is very low, etc., here will not be repeated. There are often statements on the Internet that the expansion is not good, their own tissues will grow into it, it is difficult to take, that is for the very general skills of the doctor. I want to emphasize that: this material is only compatible with human tissue, our tissues to recognize it, will grow in, only long-lasting. Good material to get the right understanding and reasonable use, expansion in the hands of experienced doctors, the infection rate is much lower than silicone, if there is no rejection can be permanently implanted, and in case of need is extremely easy to remove. It can also be used for candidates with thin skin at the tip of the nose after silicone rhinoplasty. Simple, right? But in this seemingly simple process we accomplish several important things: narrowing the nose, raising the bridge, increasing the length of the nose, highlighting the tip, narrowing the nose, lengthening the nasal column, reducing the exposure of the nostrils and bringing them closer to a long oval shape. A little shorter here, a little longer there; a little more extended below, a little higher above; no need for extremes, a few positive and a few negative, the whole nose is coordinated! Each step will be slightly altered in degree for each individual, each step should be considered in the light of the other several steps to be just right. Moreover, the joint surgery with the application of expansions usually does not have to take ear cartilage or septal cartilage to avoid unnecessary trauma. In this way, through the rectification of the nasal subunits to create a nose that fits the face shape, age, occupation and personality of the candidate just like a natural nose.
