The patient is a 64-year-old male, senior consultant of a company, who has suffered from open-angle glaucoma for more than 20 years and has been treated both domestically and internationally, and his left eye has been operated. He had been treated at home and abroad and his left eye had been operated. He had high intraocular pressure in both eyes and visual field defects. At the time of consultation, his naked vision was 0.3 in the right eye and 0.8 in the left eye. he was anxious and had no other discomfort, with a red tongue, yellow fur and thin pulse. The diagnosis: liver qi stagnation, which turns into fire, causing stagnation of qi and blood in the eyes and obstruction of the arteries. The liver disease offends the spleen, and the spleen loses its healthy function, making it difficult to excrete the sacred water in the eyes, causing the accumulation of stagnation in the eyes, resulting in increased intraocular pressure, decreased visual acuity and reduced visual field. Treatment: Dredge the liver and strengthen the spleen, invigorate the blood and open the veins. Acupuncture points: middle epigastrium, abdominal Renzhong (upper third of middle epigastrium), hypogastrium, Shui Shui, Qi Hai, Guan Yuan, double Tianshu, double Da Heng, double Shui Shui. Direction of acupuncture: middle epigastric, guan yuan, and double water channels are stabbed downward (kan); abdominal manzhong (abdominal shui gou point), lower epigastric (abdominal dazhi point), water (abdominal zhi yang point), and qihai are all stabbed upward (li). The right tianshu is stabbed downward and the left tianshu is stabbed upward. The two needles of the left great cross are stabbed to the southwest (Kun Spleen) and west; the two needles of the right great cross are stabbed to the southeast (Zhen Liver) and east (Xun Gallbladder). Acupuncture techniques: water, qihai, and the left great cross (kun spleen, tui lung) are all used in the tonic method; the right great cross (Zhen liver, xun gallbladder) is used in the diarrheal method; the remaining points are flatly tonic and flatly diarrheal. The rest of the acupuncture points are flat tonifying and flat diaphoretic. Treatments are given every other day, 10 times for one course of treatment. Treatment effect: check visual acuity: naked vision: after the first needle: right eye 0.3 improved to 0.6; left eye 0.8 improved to 1.0. after the fifth needle: right eye 1.0, left 1.0. after the tenth needle: right 1.0, left 1.0. the patient was busy with work and only one course of treatment was given. three months later the follow-up vision was maintained without decline. The pathological mechanism of glaucoma is related to the following factors: 1. It is related to the disorder of fluid metabolism. The “Su Wen – Jing Yin Bu Lun” said: “Drinking into the stomach, overflowing essence, up to the spleen, spleen temperament disperses essence, up to the lungs, through the water channel, subordinate to the bladder, water essence four cloth, five meridians parallel”, this paragraph discusses the transmission process of grain and food essence, which has become the theoretical basis for later discussions on the metabolism of fluids. The direct causative factor of glaucoma is that the atrial fluid in the eye is not properly distributed and metabolized, and stagnation is the disease, which is also a disorder of water and fluid metabolism, so it is related to the dysfunction of the lung, spleen, and the three jiao in promoting, distributing, and regulating fluid. 2.Related to atrial water flow disorder. The flow of atrial fluid from the posterior chamber into the anterior chamber is controlled by a self-control switch “valve”, the opening and closing of which is determined by the size of the pupillary sphincter tension. From the perspective of the internal organs, the spleen is the master of muscles, the liver is the master of tendons, and the kidney is the master of opening and closing, so the strength of the pupillary sphincter is related to the spleen, kidney, and liver, especially directly related to spleen and kidney yang. Because Yang Qi is active, ascending and flowing, the “Eight Difficulties” says: “Qi is the root of man, and if the root is cut off, the stem and leaves will wither”, where Qi refers to the Yang Qi of the kidney and the Yang Qi of the spleen, which is the driving force of human life activities. If the spleen and kidney Yang Qi is insufficient, the valve will be weak, open and close, and eventually lead to the flow of water obstruction, stagnation in the internal and cause disease. 3. It is related to the amount of atrial fluid produced. Room water is fluid, its operation must be clear Yang rise and promote. The liver is a rigid organ, the main rising active, liver qi through the spring, for the beginning of growth and development. Therefore, the rising and rising of liver qi is in line with the clear yang of the eye orifice. It is the driving force behind the production of room water. Therefore, the amount of atrium water produced is closely related to the liver. In addition, the liver is the master of drainage, the atrial fluid maintains normal movement without stagnation, which also depends on the drainage of liver qi. In view of the above pathological mechanism, a treatment method was established based on the principles of supporting Yang and helping Yang (Yang of the spleen and kidney), draining the Liver, benefiting the Lung, and regulating the three jiao. The middle epigastric and abdominal Renzhong, the latter being equivalent to the holographic abdominal Shuigou point, are stabbed in two directions, i.e., Li and Kan, respectively, with one meaning: one yin and one yang, yin and yang intersecting, water and fire both helping to prevent eye pressure from rising; the other meaning: awakening the mind and opening the orifices, the main clearing the mind can guide the qi, the qi can flow smoothly and the eyes can see with blood. The hypochondrium is equivalent to the full breath abdominal large vertebrae point, stabbed to leave, intended to pass through the Governor’s vein, into the brain, camping the House of the Yuan Shen. Combined with modern medicine, it can be considered to increase the blood flow to the brain through the vertebrobasilar artery thus nourishing the eye system. The water component corresponds to the point of Zhiyang on the abdomen of the whole body, which is also pricked to leave, and is intended to nourish the heart, strengthen the heart and increase the blood flow in the coronary arteries. Qihai is pricked upward to nourish the yang energy of the body. Tianshu is located in the mid-abdomen, bearing the top and the bottom, and the direction of acupuncture is left ascending and right descending. This comes from the Bagua diagram, in which the left is the Spleen, which is responsible for ascending the Clear; the right is the Stomach, which is responsible for descending the Cloud. With this combination, the qi can be better regulated and the flow of qi and blood in the middle jiao can be mediated. (2) According to the theory of interconnectedness of the internal organs, the kidneys are connected to the three jiao, and together with the sink, water, and water channels (each of which is responsible for the upper, middle, and lower jiao), the evil of phlegm and water-drinking can be channelled down through the water channels of the three jiao, which can lower eye pressure. Double stabbing of the liver and gallbladder on the right side of the great cross has the following meanings: (1) to reduce the amount of atrial fluid production; (2) to stop spasm, open valves, pass atrial fluid and lower intraocular pressure. (1) The lung is connected to the bladder, so it has the function of lifting the pot and uncovering the lid and facilitating urination; (2) it can strengthen the spleen to transport water and dampness.
