Bilirubin encephalopathy aka nuclear jaundice (late final stage) I. Clinical: neurological syndrome (in four phases) 1. Warning phase (0-12hr): marked increase in jaundice, drowsiness, reduced sucking, hypotonia; 2. Convulsive phase (12-24hr): staring, hyperthermia, high pitched cry, convulsions, corns, expiration, pulmonary hemorrhage, death; 3. Recovery phase (after 2 days): gradual recovery; 4. Sequelae period (after 2 months): mental retardation, cerebral palsy, tetralogy of Fallot (tardive dyskinesia, oculomotor disorders, auditory disorders, dental enamel hypoplasia). II. Pathology Specific areas of brain tissue (hippocampus, pallidum, basal ganglia) are stained yellow with unconjugated bilirubin, causing neurotoxic effects.
