Pituitary hypothyroidism, also known as central hypothyroidism, is treated with oral levothyroxine tablets, just like primary hypothyroidism. The goal of treatment for pituitary hypothyroidism is to restore blood levels of thyroid hormone (FT4) to within the normal range, while blood levels of thyroid stimulating hormone (TSH) do not necessarily return to normal. The starting dose and the time needed to reach the full replacement dose are determined by age, weight and heart status. Patients younger than 50 years old with no previous history of heart disease can reach the full replacement dose as soon as possible; before taking levothyroxine tablets in patients over 50 years old, the heart status should be routinely checked. Generally start with 25-50ug/d and increase by 25ug every 1-2 weeks until the therapeutic goal is reached. When supplementing thyroid hormone, thyroid hormone levels usually need to be rechecked once every 4-6 weeks. Based on the results of the thyroid hormone level check, the dosage of levothyroxine tablets will be adjusted until the treatment goal is reached. After the treatment goal is reached, hormone levels need to be rechecked every 6 months to 1 year.
