What are the advantages of endoscopic management of bile duct stones?

       Gallstone disease occurs in almost 10-20% of the world population, and 30% of these stones are located in the bile ducts. Bile duct stones are of greater clinical importance than gallbladder stones because they can lead to a range of serious complications such as cholangitis, pancreatitis, and obstructive jaundice. These serious complications occur more often in advanced age. Surgical epidemiological studies have found that the detection rate of intraoperative bile duct stones increases with age, from about 5% up to 30 years of age, to 15% between 31-60 years of age, and up to 45% between 61-80 years of age.  Since the introduction of papillotomy technique to remove bile duct stones by West German and Japanese physicians in 1974, there have been many new technological advances, such as mechanical lithotripsy, fluid electrolysis, laser lithotripsy, etc., which have significantly expanded the scope of endoscopic treatment of bile duct stones, such as giant stones, embedded stones, Mirizzi’s syndrome, making endoscopic approach to bile duct stones the main and preferred treatment for gallstone disease .  The advantages of endoscopic treatment of bile duct stones are: 1) high success rate; 2) low complications and mortality; 3) no need for general anesthesia; 4) short hospital stay; 5) low medical costs.  These advantages make endoscopic treatment the preferred method.  In the treatment of bile duct stones, the first consideration should be how to safely and completely remove bile duct stones with less pain to the patient and a low rate of postoperative complications. The advantages of the endoscopic approach described above are readily accepted. In fact after more than 30 years of practice, the vast majority of patients with biliary stones treated and handled by the endoscopic approach worldwide have achieved complete and safe stone removal by endoscopic surgery.  Endoscopic removal of stones followed by complementary herbal treatment can better prevent and treat stone re-formation.
