First of all, the incidence of varicose veins is very high, even up to 30% in foreign statistics. As humans evolved from reptiles to upright, the height of the heart from the ground increased, resulting in higher venous pressure in the lower extremities, which is the cause of varicose veins and an evolutionary result that cannot be changed. Secondly, it is only a matter of time before varicose veins will undoubtedly eventually produce complications that will seriously affect the quality of life. Varicose veins are produced by venous hypertension, and varicose veins in turn aggravate venous hypertension, a vicious circle that eventually leads to skin dystrophy, itching, eczema, and skin ulcers. I often meet patients in the clinic who say, “My friend (or relative or acquaintance) has more varicose veins than I do, and they are all bulging, but I don’t see anything wrong with them”. In fact, most people see a point in time of the disease, but not the final outcome, while as a vascular surgeon, they see more the final outcome of the disease. By far the oldest varicose vein patient treated in our department was an 88 year old man with a severe venous ulcer. As you can imagine, he was definitely reluctant to have surgery if he could do without it, and was forced to go to the hospital only when it finally did not work. At this time, there are many other examples of increasing age and serious complications that make the surgery more difficult and raise the risk of surgery. Finally, psychologically speaking, most patients are reluctant to choose the more invasive surgical treatment. Domestic physicians often believe that the well-developed medical systems and better educated patients in foreign developed countries are bound to treat varicose veins early. However, from my experience abroad, foreign patients are just as reluctant to undergo the more invasive surgical treatments. I myself have early varicose veins due to long term surgical standing and from my heart, I am also reluctant to undergo traumatic surgical treatment. To sum up, the following conclusion is drawn: varicose veins have a high incidence and are harmful, but most patients are unwilling to accept the highly invasive surgical treatment although it is effective. There is only one way out to solve this problem, and that is: individualized minimally invasive treatment. At present, the more effective methods for treating varicose veins are 1.conservative treatment with drugs and elastic stockings: it is suitable for the early stage of varicose veins to prevent the deterioration of varicose veins and solve the symptoms of swelling of lower limbs; 2.traditional high saphenous vein ligation and trunk stripping surgery, which has clear treatment effect and is more traumatic; 3.minimally invasive treatment: it is generally accepted at home and abroad that the endovenous laser closure treatment and endovenous radiofrequency closure Treatment, good efficacy, less trauma; 4, sclerotherapy, preferably using foam sclerotherapy, minimal trauma, but easy to recur. Individualized minimally invasive treatment plan: combined with the specific condition of the patient, the least traumatic comprehensive treatment plan, so that the efficacy and good, and the patient is easy to accept.
