There are so many patients suffering from gallstones that laparoscopic removal of the gallbladder, the so-called minimally invasive surgery to remove the gallbladder, has become the gold standard for surgical treatment of gallbladder stones, and this treatment has also been accepted by the general public. Because of this, routine removal of the gallbladder has become the choice of many general surgeons today for the treatment of gallbladder stones, without exception, so are there any gallbladders that can be preserved in these removed patients? Are all gallbladders with stones removed indiscriminately? It is worthwhile for each treating physician to seriously reflect on the fact that each gallbladder should be treated with care and should be cherished by each patient. As a patient, we should also raise the awareness of cherishing our own gallbladder and not to give up our gallbladder at will. For patients with heavy chronic inflammation, frequent recurrent attacks, many stones, fragmentation, and easy recurrence, we should not be appeased and should remove these gallbladders, thus removing the breeding ground for the disease and avoiding recurrent attacks. For those patients with good gallbladder function, mild inflammation of the cyst wall, small stones or single stones, should we strive to preserve the gallbladder? Especially for patients who are eligible for gallbladder preservation and have a strong intention to preserve gallbladder, we should be more merciful and remove the stones while leaving the gallbladder, so that the patient can be less damaged and more gallbladder function can be preserved, thus minimizing the physiological and psychological impact of surgery on the patient. Of course, whether the gallbladder can be retained depends on several factors, firstly, the condition of the gallbladder and stones, which is an objective factor and the most important factor; secondly, the doctor’s understanding of biliary preservation; and the patient’s knowledge and intention of biliary preservation, which is also a factor that cannot be ignored. There are clear advantages and disadvantages of biliary preservation, and there are also unavoidable hidden dangers, so the choice depends on the three main factors mentioned above. At the same time, patients should also cherish their gallbladders and not make a rash decision to part with them. Therefore, when treating gallbladder stones, there should always be a philosophy: please cherish each of our gallbladders.
