Case: The patient, female, 73 years old, from Guangzhou, Guangdong, was treated in an external hospital in May 2009 for swelling of both lower limbs, nausea and vomiting, chills in the extremities, and vaginal bleeding, which did not improve significantly, but continued to deteriorate. It was a hale and hearty old lady, but in a week or so she became depressed and looked like two people. It turned out to be a tumor After further examination, it was found that the cervical mass had invaded the posterior wall of the bladder and the end of the ureter, resulting in ureteral obstruction, bilateral hydronephrosis and subsequent renal insufficiency, and was diagnosed as cervical cancer with stage IIIb. How to solve The patient was in advanced stage of cervical cancer, invading the bladder and ureter, resulting in fluid accumulation in both kidneys and renal insufficiency, making further surgery impossible, but the tumor was growing day by day, the patient could not urinate and her kidney function was deteriorating day by day. When the patient and his family were at a loss, the patient was referred to our radiotherapy department for treatment. Director Ren Jun firstly performed bilateral percutaneous nephrostomy for the patient, and combined with Chinese medicine to warm the Yang and transform the Qi, the prescription was Wu Ling San plus and minus. The patient’s condition was immediately relieved, and not only did she not have swelling in both lower limbs, but also her appetite and spirit were significantly restored in all aspects. In the process of the old lady’s kidney function recovery, Director Ren Jun carefully made radiotherapy for the patient, and with the concurrent chemotherapy, the patient’s vaginal bleeding symptoms gradually disappeared, but began to nausea and vomiting, loss of appetite. The old lady always ordered two meals every day, one first, and after she finished eating, she might vomit and vomit what she just ate, but after a break, she would always take out another one and continue eating until she didn’t vomit. Most of the patients have a fear of radiotherapy and chemotherapy, so when chemotherapy was mentioned, the old lady was afraid of it, so the day before the chemotherapy, before the chemotherapy drug was used, the old lady started to vomit and become nauseous. But Director Renjun repeatedly explained to the patient and family members that radiotherapy is one of the most important means of treatment for middle and late stage cervical cancer, which is effective and inexpensive. And after the treatment of adjuvant therapy, the toxic side effects of gastrointestinal tract can be effectively controlled in more than 90% of patients. Treatment effect: Under the careful arrangement of Director Ren Jun, the patient successfully completed the treatment. In particular, from the third week of radiotherapy, the old lady began to relax and reduce her psychological burden, her gastrointestinal symptoms were reduced day by day, vaginal bleeding gradually disappeared completely, the old lady basically regained her appearance before the disease, and gradually started to urinate on her own after 2 months, the patient praised the miracle of radiotherapy, and the gynecological ultrasound and pelvic MRI tumor completely disappeared after 3 months. The patient got a perfect treatment effect. The old lady’s children were full of praise. What is cervical cancer: Cervical cancer is one of the most common malignant tumors in gynecology. Globally, more than 200,000 women die of cervical cancer every year. In developing countries, cervical cancer tops the list of common and frequent gynecological tumors. The number of new cases detected in China is 131,500 per year, with the highest mortality rate in Shanxi and the lowest in Tibet. Cervical cancer is a malignant tumor that occurs in the vaginal part of the uterus and the cervical canal. The general trend is that rural areas are higher than urban areas and mountainous areas are higher than plains. According to the retrospective survey of provinces, cities and autonomous regions, the mortality rate of cervical cancer in China accounts for the first place of total cancer mortality and the first place of female cancer. It can also metastasize through lymphatic vessels to the paracervical, internal iliac, external iliac, and inguinal lymph nodes, and in advanced stages even to the supraclavicular and other lymph nodes throughout the body. Hematogenous metastasis is relatively rare, and the common metastatic sites are lung, liver and bone. When the symptoms of cervical cancer appear three months later, 2/3 of the patients are in advanced stage of cancer What is radiotherapy: Radiotherapy is radiation therapy, commonly known as “electrotherapy”. It refers to the use of radiation to eliminate the lesions. Radiotherapy is an important tool for treating malignant tumors, and it can produce better results for many cancers. Due to the biological effect of radiation, it can kill the cancerous tissues in the largest amount, destroy them and make them shrink. However, radiotherapy can also produce many toxic side effects such as radiation dermatitis, radiation esophagitis and loss of appetite, nausea, vomiting, abdominal pain, diarrhea or constipation. In particular, radiation dermatitis is the most common. The so-called skin reaction, in common parlance, means that the skin is baked black, baked red, or even baked rotten. How to prevent and mitigate gastrointestinal reactions: How do we mitigate gastrointestinal reactions? The common clinical methods include: (1) maintaining healthy habits of gastrointestinal tract, regular bowel movement, regular meals, and paying attention to meal hygiene to prevent the possibility of eating bad stomach during the treatment (2) taking some drugs to regulate the function of gastrointestinal tract, such as Gastrodia, etc. (3) there are also some topical Chinese medicine packs, such as Wu Ju Ju Hot Am package applied topically, etc. In the treatment plan of cervical cancer patients, radiotherapy is of course the most important. However, the management of gastrointestinal reactions is also a condition to ensure that radiotherapy can be carried out successfully. This patient was cured after comprehensive treatment while minimizing the reaction to radiotherapy (especially gastrointestinal reaction). We also hope that better and more advanced treatments will be available in the future to relieve cancer patients from their illnesses.
