Intercostal neuralgia, also known as intercostal neuritis, is a group of symptoms that refer to damage to the thoracic nerve roots (i.e., intercostal nerve) due to different causes, such as: thoracic spine degeneration, thoracic spine tuberculosis, thoracic spine injury, thoracic spine duralgia, tumor, ankylosing spondylitis and other diseases or rib, mediastinal, pleural lesions, intercostal nerve compression by the above diseases, stimulation, inflammatory response, and the appearance of the chest intercostal or abdominal band-like pain. Intercostal neuralgia is a syndrome of pain in the intercostal region of the chest or abdomen. Intercostal neuralgia is a syndrome of frequent pain in one or more intercostal areas with episodes of exacerbation. Primary intercostal neuralgia is rare, but secondary cases are usually associated with viral infection, toxin stimulation, mechanical injury, and foreign body compression. The nature of the pain is mostly stabbing or burning and is distributed along the intercostal nerve. Symptoms: Intercostal neuralgia is divided into two types: secondary and primary, and can be secondary to thoracic degeneration, thoracic tuberculosis, thoracic spine injury, thoracic duralgia, tumors, ankylosing spondylitis and other diseases; dry intercostal neuralgia can be secondary to rib, mediastinal or pleural lesions. Primary intercostal neuralgia is rare. At the onset of intercostal neuralgia, the pain is seen to radiate in a semi-circular pattern from posterior to anterior along the corresponding intercostal space; the pain is stabbing or burning. The pain increases with coughing, deep breathing or sneezing. The pain is usually located in one of the nerves on one side. Physical examination reveals significant pressure pain in the paraspinal process and intercostal space of the thoracic spine; typical patients with radicular intercostal neuralgia have a positive flexion test; the distribution area of the affected nerve often shows neurological impairment such as sensory hypersensitivity or hypoesthesia. The treatment and prevention treatment should clarify the primary lesion and use appropriate treatment methods. You can use drugs, physical therapy, acupuncture, massage, etc. (a) body acupuncture 1, acupuncture points: the main point: the sea, Qiu Hui, Qu Chi. Supporting points: Zhigou, Yanglingquan, Hua Tuo (the corresponding segment of the lesion), Ligou. 2.Treatment The main acupuncture point is only one point at a time, if the effect is not obvious, add supporting points. Shining sea, take both sides, with inhalation into the needle 1 inch to 1.5 inches, repeated twisting combined with lifting and inserting, lasting 3 minutes to 5 minutes, apply the diarrhea method, retain the needle for 15 minutes. Qiu Hui point, it is advisable to take the left side of the disease, the right side of the disease, the left side of the disease, the same acupuncture method as Shouhai. Quchi, take the acupuncture point on the healthy side, after acupuncture, twist with a small amplitude and high frequency, and ask the patient to take deep breaths and massage the affected area. Retain the needle for 15 minutes. Huatuo pinch spine, deep acupuncture so that the needle sensation along the intercostal nerve radiation, only the affected side of the point, the application of the flat tonic and flat diarrhea method, the rest of the points are taken bilaterally, the same acupuncture method as the sea. The acupuncture method is the same as that of Shouhai. 3, the efficacy of the efficacy assessment criteria: cured: pain completely disappeared; effective: pain reduction of two-thirds; improved: pain reduction of one-third to one-half; invalid: pain is not obvious. (B) Skin acupuncture 1. Main acupuncture point: Huatuo Jijiao point, the back line of the bladder meridian. Supporting points: the intercostal space of the lesion area, the wakefulness of the bile meridian. 2.Treatment After routine disinfection, the patient is given 5 repetitive medium-intensity percussions from neck 7 to neck 8 of the Hua Tuo Pinnacle Point and from Da Loom to Guan Yuan Yu of the Bladder meridian, and the patient is cupped with the flash fire method for 10 minutes. The patient is then placed on his side, with the affected area facing upward, and the patient is tapped 5 times along the rib space of the lesion area and along the circumferential section of the bile meridian. The patient then percusses one rib space above and below the painful area for 2 times. Use medium-intensity techniques. After tapping, cupping in the most obvious area of pain for 10 minutes. Once a day, 10 times for a course of treatment. (C) Cupping 1. Main acupuncture point: A-Yi point. Supporting points: Neiguan, Yanglingquan. Location of A-Yi point: the most obvious place of pain. 2.Treatment Make the patient lie on his back, first take the supporting points for acupuncture, twist the needle into the method of acupuncture Nei Guan, after the sensation of stimulation with the lifting method to increase the amount of stimulation, so that the needle sensation radiates to the upper arm. At the same time, the patient takes a deep breath and deeply stabs Yanglingquan, penetrating directly to Yinlingquan. After induction, use the twisting method to increase the amount of stimulation, so that the needle sensation can be transmitted upward and downward. Retain the needle for 30 to 50 minutes, and perform acupuncture every 5 minutes. Then, at the A-Yi point, where the pain is most obvious, after routine disinfection, take the skin needle and tap from light to heavy, tap until the skin is red, and then absorb it with the flash fire method or cast fire method, or use the vacuum cupping device to suck and cupping. Leave the cupping for 10 to 15 minutes and remove the cupping when the skin is purplish red with blood stasis. Treatment every other day, 6 times a course of treatment, the treatment interval of 3 days to 5 days. (iv) electroacupuncture 1, acupuncture points Main acupuncture points: the period of the door, the branch gou, Yanglingquan, foot three li. Supporting points: Taichong, Zhiyang, Liver, Kidney, Xingma, Qiushu. 2.Treatment Take 2-3 main points and 1-2 supporting points. After obtaining the qi, apply the twisting technique, first tonic and then diarrhea, run the needle for 10 minutes, then pass the electro-acupuncture, the negative pole is connected to the main point, the positive pole is connected to the matching point. Use dense or sparse waves, as strong as the patient can tolerate, and energize for 5 to 20 minutes. Once a day.
