Focused ultrasound treatment of female vulvar inflammatory disease and cervical columnar epithelial heterogeneity

VINN (vulvar intraepithelial non-neoplastic lesions), formerly known as vulvar skin dystrophy or leukoplakia, is a group of chronic diseases in which pigmentation changes occur in the skin and mucosal tissues of the female vulva, characterized by itching and burning pain in the vulva, hypopigmentation or whitening of the skin and mucosal membranes in the lesion area, and skin changes such as roughness and cracking of the skin. In the late stage, local atrophy, narrowing of the vaginal opening, reduced elasticity and painful intercourse may occur, seriously affecting the quality of life of patients. Simple pruritus vulvae (SPV) mostly originates from previous vulvovaginal inflammation and is characterized by itching and discomfort in the vulva, with no significant changes in the local phenotype other than congestion and scratch marks. Currently, it is mostly treated with hormones, local medications such as Chinese herbs or physiotherapy, with varying degrees of effectiveness. Surgical excision of vulvar lesions or simple vulvectomy can be effective for a while, but the recurrence rate is up to 50% or more, and the postoperative vulvar scar contracture seriously affects the physical and mental health of patients and the quality of sexual life. The principle of HIFU treatment for SPV, LSV and SHV is to use the good tissue penetration and energy deposition of ultrasound to act on the dermis of the lesioned skin, improve local blood circulation, and the lesioned tissue undergoes functional changes under the effect of heat, mechanical effect and cavitation effect of ultrasound, increasing the permeability of the cell membrane. Promote cell proliferation, promote the formation of blood vessels and peripheral nerves, tissue repair and regeneration, so that the lesioned tissue to restore the normal form and color. Thus, the therapeutic effect is achieved. Since the local tissues appear transient congestion and edema after ultrasound treatment without the tissue repair process of crusting and shedding of other treatments, the treatment area can keep the skin intact. Even if there is a recurrence, a better result can still be achieved by re-treatment with HIFU. The treatment principle of CCE is to gather low-energy ultrasound on the surface of the lesion, so that the temperature of the tissue in the target area suddenly rises to 55-65 ℃, until the target area of the lesion tissue completely thermal coagulation and destruction and play a therapeutic role, there is no significant damage to the surrounding tissue. After a lot of research and treatment of many patients, we have obtained satisfactory results. For some patients with vulvar itching symptoms previously treated repeatedly by drugs with no obvious effect, after HIFU treatment significantly reduced the total efficiency of 90. 4%, patient satisfaction significantly increased. The white lesion area of vulva gradually approached normal color or pink and skin elasticity softened or returned to normal, and no change was seen after treatment. The apparent efficiency of the LSV group was 94.8% and that of the SHV group was 92.0%. The cure rate for all levels of CCE reached 100%. The unique features of this method compared with other physical therapy are: less treatment times, shorter single treatment time, no coking material, safe and non-invasive, high accuracy, obvious efficacy, and satisfactory recovery of vulva morphology and color.
