Hypertension is a syndrome mainly manifested by elevated blood pressure in the body circulation, and is an important cause and risk factor of many cardiovascular and cerebrovascular diseases. There is no name for hypertension in Chinese medicine, but according to the main clinical manifestations and its development, it can be classified as “dizziness”, “headache” and “liver wind” in Chinese medicine. Prof. Shi Dazhuo treats hypertension from the perspective of harmonizing yin and yang and promoting the flow of qi and blood, and has obtained good clinical results. The pathogenesis of hypertension is complex, but Chinese medicine believes that it is related to the physical endowment. Old age, physical weakness, excessive exertion, internal injury to the essence; emotional and mental disorders, the five will be too extreme, liver Yang into fire; diet, too much fat, sweet, salty, spicy wine and mash, resulting in phlegm and dampness, can lead to the internal organs of the imbalance of yin and Yang Qi and blood and the development of the disease. Hypertension, marked by elevated blood pressure, is characterized by headache and dizziness as the main clinical symptoms. It should be noted that headache and dizziness are not the same as hyperactivity of liver and yang or yin deficiency and hyperactivity of yang, and elevated blood pressure should not be equated with hyperactivity of liver and yang. In clinical hypertension, there are deficiencies and realities: the deficiencies are liver and kidney yin deficiency, kidney essence deficiency, spleen qi deficiency, and kidney loss of warmth; the realities are phlegm, blood stasis, qi stagnation, liver yang, and liver fire. Clinical hypertensive patients, mostly occurring after middle age, often have symptoms such as soreness and weakness of the waist and knees, intolerance to exertion, frequent nocturnal urination, shortness of breath and weakness, and deficient and floating complexion, especially in obese hypertensive patients. These are all manifestations of “internal seizure of essence”, failure of Qi to transform essence, and internal stagnation of phlegm and dampness. Liver yang disturbing the clear orifices can lead to headache and dizziness; insufficient essence, loss of filling of the medulla, insufficient blood, loss of nourishment of the cerebral vasculature, insufficient qi, sinking of the clear yang, blockage of phlegm, failure of the clear yang to rise, stasis of blood, and unfavorable qi and blood can also lead to dizziness and headache. It can be seen that headache and dizziness have various pathogenic mechanisms, but the most fundamental pathogenic mechanism is the imbalance of yin and yang and the loss of harmony between qi and blood. Therefore, in clinical treatment, we should identify the deficiency and realities in detail, harmonize yin and yang, and smooth the qi flow, so that the qi flow can rise and fall in a controlled manner, yin and yang can be harmonized, and blood vessels can be facilitated, and the symptoms can be cured. According to the characteristics of the pathogenesis of hypertension, which is the imbalance of Yin and Yang, Qi and Blood, we should follow the principles of “the one who is deficient should be tonified”, “the one who is damaged should be benefited”, “the one who is high should be suppressed”, and “the one who is frightened should be pacified”. “The main treatment method is to harmonize yin and yang, and to smooth the flow of qi and blood. 1, calm the liver and submerge Yang: hypertensive patients, most of them have atherosclerosis (AS), AS patients mostly have the manifestation of string pulse, but the string pulse alone can not be considered as hyperactive liver Yang, must be accompanied by headache, dizziness, red face, anxious temperament, red tongue, only then can be identified as hyperactive liver Yang. In hypertensive patients with hyperactivity of liver yang, do not abuse or overuse products that suppress liver and subdue yang, such as dragon bone, oyster, mother of pearl and ochre, etc. These drugs are heavy sedatives that hinder the liver from regulating the flow and easily suppress the spleen and stomach from receiving and transporting. The liver should be softened and dredged, the qi and blood harmonized, and the liver pacified and submerged. To soften the liver, use Bai Shao, Sheng Di, Cornu Cervi Pantotrichum, Mai Dong, Chai Hu, Su Zi, Citrus Aurantium, etc. To harmonize the Qi and Blood, use Chuan Knee, Angelica Sinensis, Dan Shen, Chuan Xiong, etc. If the liver yang is hyperactive, use chrysanthemum, peony, Xia Gu Cao, hooked vine, and di dragon; if the liver yang is hyperactive, use ocher, mother of pearl, and raw oyster. The liver is a rigid organ, the body is Yin but Yang, too much use of submerged town, but increase its rigidity. Therefore, even if Liver-Yang is hyperactive, when the liver is pacified, it should be accompanied by Yin Chen, maidenhair, mulberry leaf and Chai Hu, etc., which are used to dredge the liver and elevate it, in order to follow the nature of the liver and to ascend before descending. After the liver yang surge is relieved, the method of harmonizing yin and yang qi and blood should be used. 2, invigorate blood circulation and remove blood stasis: hypertension is a vascular disease, vascular tension increases, blood pressure rises. Modern research shows that hypertensive patients have increased vascular tone, increased platelet activity, easy adhesion and aggregation. Chinese medicine considers these pathological changes as liver wind and blood stasis. Therefore, the treatment should be combined with blood-stasis activating drugs. Modern pharmacological studies have shown that many blood-stasis activators such as Danshen, Dampi, Dilong, Red Peony, White Peony and Tiger Balm have certain antihypertensive effects. In addition, blood-stasis activating drugs can dilate peripheral blood vessels, reduce vascular resistance, improve glomerular filtration rate, and increase water and sodium excretion. These drug effects are considered by modern medicine to be beneficial to lowering blood pressure. TCM believes that qi and blood are interdependent, blood carries qi, blood vessels are harmonized, and the hyperactive yang is easy to calm and submerge. For the treatment of hyperactive yang in TCM, it is advisable to use blood-activating and stasis-transforming drugs that induce blood to return to the meridian and induce blood to move downward, such as Chuan Niu Kne and Angelica Sinensis. In addition, according to the difference of deficiency, actuality, cold and heat in the clinical evidence, herbs to invigorate the Blood and resolve stasis should be selected and applied: for those with hyperactivity of Liver-Yang, use Dan Pi, Di Long, Chuan Niu Knee, Red Peony, etc. to cool the Liver and invigorate the Blood and draw the Blood downward; for Yang deficiency and cold stagnation or internal blockage of cold dampness, use Red Flower, Gui Zhi, Chuan Xiong, Tablet Ginger Huang, etc. to warm the Blood and help Yang Qi to rise, condense the cold and warm it to disperse; for Blood deficiency, use Angelica Sinensis, Chicken Blood Vine, Dan Shen, etc. to nourish the Blood and invigorate the Blood; for edema, use For those with oedema, use blood-boosting and water-relieving herbs such as Radix et Rhizoma Zeleniae, Radix et Rhizoma Yi, and Radix et Rhizoma Chrysanthemum. 3, ascending Yang dispersing wind: in the treatment of hypertension in Chinese medicine, the correct use of ascending Yang dispersing wind medicine such as Chai Hu, Ge Ge Ge, Manchurian, Qiang Wu, Fang Feng, and warm Yang medicine such as Pei Zi, Gui Ji, Cinnamon, etc., for phlegm and turbidity in the blockage, clear Yang does not rise, or Yang deficiency can not transform the qi to rise clear patients, can often receive good results. Do not stick to the warmth and dispersion easy to make “Yang move up” and bound clinical use of drugs. The basic pathogenesis of hypertension is the loss of harmony between qi and blood, yin and yang, rather than hyperactivity of liver and yang. When the pathogenesis of hypertension is Yang deficiency and Liver Yang does not rise, the use of warming Yang medicines such as Xian Ling Spleen, Ba Ji Tian, Gui Zhi, etc., especially in patients with Kidney Yang deficiency, allows Yang to rise and Liver Qi to flow, restoring its Qi and Blood rushing and harmonizing nature, blood pressure can often be restored. In patients with hypertension, if the pathogenesis is spleen deficiency, phlegm and turbidity blockage, or if liver qi is stagnant and not well-developed, the use of Chai Hu, Fang Feng, Manchurian, Pueraria Mirifica, Qiang Wu, etc., can dredge the liver and smooth out the qi flow; secondly, it can make clear yang rise and phlegm and turbidity dissolve, which is very beneficial to the disease. Modern pharmacological studies of Pueraria lobata have confirmed that it can dilate blood vessels, improve myocardial blood supply, and have β-blocking effects, which can cause blood pressure to drop [5]. For hypertensive patients with deficiency of kidney essence (qi) or kidney yang, deficiency of liver yang, headache and dizziness, Right Return Pill or Eight Flavors Kidney Qi Pill with whole tianma, dulcimer, Chuanniu Knee, Pueraria Mirifica and Fanfeng can be used to treat them. For spleen deficiency and dampness, phlegm and turbidity in the blockage, clear Yang does not rise, can be used Xiang Sha six Jun Zi Tang combined with half Xia Bai Zhu Tian Ma Tang, heavy use of Tian Ma treatment, Tian Ma can be used to more than 30 grams. Tianma body fat and soft, taste although pungent but not ascending and dispersing, although sweet and not nourishing, all the evidence of dizziness of the head and eyes, can be applied with the evidence. 4, reconciliation of the punch and any: women’s menopause or post-menopause hypertension, mostly related to the punch and any disharmony. The cause of the disharmony of the ramifications comes from the imbalance of the yin and yang of the kidney. It can be treated with Erxian Tang plus Chuan Niu Knee and Ge Gen. The two medicines, Chuan Niuxi and Ge Gen, are combined, one into the Qi and one into the Blood, to harmonize the Qi and Blood. Third, pay attention to both add, flexible addition and subtraction need to add a small hat 1, add phlegm-liquidating drugs “Danxi Xinfa? Dizziness of the head”: “No phlegm is not dizziness”. Hypertension is mostly seen in middle-aged and elderly people, liver and kidney yin deficiency, yin does not converge yang, liver yang is hyperactive, and it can attack the spleen and earth, which can lead to spleen loss of health, phlegm and dampness; kidney is the source of water, kidney yang deficiency, can not warm the five organs, water transmission and distribution metabolism is not normal, stagnant into dampness into phlegm. The phlegm and dampness formed by the above-mentioned reasons, stagnating in the veins, can lead to stagnation of qi and blood, causing stasis due to phlegm, phlegm and stagnation of blood, which will stay in the whole body with the meridians and can lead to disharmony of blood and veins and stagnation of water, thus causing an increase in blood pressure. Therefore, phlegm and watery fluid are common pathological products of hypertension. In the treatment of hypertension disease, it is often necessary to add drugs to resolve phlegm and water, such as hemihsia, atractylodes, chenpi, guavas, allium, poria, poria, zedoary, etc., with obvious effect. The liver is the organ of wind and wood, and the internal fire is sent to the phase, the liver yang is hyperactive, the hyperactive pole turns into fire and generates wind, the wind rises and fire moves, disturbing the heart and mind, or the mood is not satisfied, the liver is depressed, the depression turns into fire and disturbs the heart and mind, which can make the mind restless and cause insomnia; the kidney water is insufficient, the liver and kidney yin is deficient, which can not help the heart, and the heart and kidney do not intermingle, which can also cause insomnia; the spleen and stomach are weak, the phlegm is blocked, the clear yang does not rise, the cloudy yin does not descend, the depression turns into fire, and the phlegm and fire Therefore, insomnia is one of the most common manifestations of hypertension. The Su Wen? The Five Viscera Generation Chapter: “When a person lies down, the blood returns to the liver, which is distributed to the five viscera, sprinkled in the six internal organs, and the yang enters the yin”. Due to blood congestion, depression and heat, coupled with insomnia, dreaming, etc., the blood returned to the liver day after day to leave its heat, accumulate micro into the focus. If heat consumes yin, yin will become more deficient and yang will become more abundant. Therefore, if a patient with hypertension rests and sleeps well, his blood pressure is relatively stable; on the contrary, if he rests and sleeps poorly, his blood pressure will rise. This shows that insomnia is the pathological result of hypertension and can further affect blood pressure, and the two are causally related. Therefore, the treatment of hypertension often requires the addition of heavy sedative drugs, such as magnet, amber, dragon bone, oyster, mother of pearl, etc., which can significantly enhance the effect of lowering blood pressure. Hypertension is a common clinical disease, in its pathological evolution, yin and yang deficiency and reality can change depending on the stage of the disease process. Generally speaking, the early stage of the disease is dominated by real evidence, but not at the end of hyperactivity of liver and yang, but also common in phlegm and turbidity, blood stasis blocking the veins, disharmony of the rhythm, liver qi stagnation, etc. The treatment should be to pacify and submerge the hyperactivity; to dry and dissolve the phlegm obstruction; to open and activate the blood stasis blocking the veins; to regulate and harmonize the discordance of the impulsive organs; and to reach and de-stagnate the liver stagnation. In the late stage of the disease, deficiency is the main cause and deficiency is mixed with reality. According to the deficiency of qi, blood, yin and yang, the methods of seeking yang in yin, seeking yin in yang, tonifying qi and blood, tonifying qi and helping yang are used. In addition, the nature of the disease is combined with the nature of the evil, using the corresponding method to eliminate evil. In conclusion, the pathogenesis of hypertension lies in the imbalance of yin and yang and the loss of harmony between qi and blood, rather than in the hyperactivity of liver and yang. If we harmonize yin and yang, and smooth the flow of qi and blood, and treat those who are ascending with descending, those who are trapped with ascending, those who are knotted with dispersing, those who are cold with warmth, and those who are hot with cold, so that yin and yang can be harmonized and qi and blood can be rushed to harmony, then all the symptoms of hypertension can be cured.
