The tendons and veins can not be restrained and can not hold, the wind of the image. …… also have the head moving but the hands and feet do not move, …… head does not move.” The cause and mechanism of the disease, the generations of doctors believe that it is due to deficiency of liver, spleen and kidney, the brain marrow by the loss of nourishment and other reasons, resulting in phlegm and stagnation of internal wind disturbing the brain marrow caused by the disease, so the treatment to calm the liver and quench the wind, nourish the liver and kidney, benefit essence and fill the marrow, spleen and kidney, etc., the emphasis on the use of pacifying the liver and submerging Yang, benefit essence and fill the marrow to replenish the Qi products. The author found in long-term clinical practice that this type of patients in the elderly, regardless of fat, thin, tall or short, have different degrees of fat tongue, white or yellowish tongue coating, slippery pulse and other signs of spleen deficiency and dampness, phlegm and heat moving wind, which is related to the decline in the function of the internal organs of the elderly, especially the spleen and kidney function is prominent. The kidney is the origin of the congenital, and the spleen is the origin of the acquired. If the congenital kidney is insufficient, the warming and pushing effect is reduced, then the spleen yang loses its warming effect and the healthy movement is out of duty; if the spleen loses its healthy movement, the biochemical source of qi and blood is lacking, resulting in the upper not being able to honor the brain and marrow, the lower not being able to warm the vital gate, and the outer not being able to moisten the tendons and veins of the limbs, then the fluid does not turn into water and generates phlegm, and the blood loses its warming effect and becomes stagnant. Therefore, as a chronic disease that requires long-term treatment, PD is treated from the spleen, not only to remove phlegm and extinguish the wind, and to cultivate the essence, but also to protect the qi of the spleen and stomach, so as to prevent the prolonged use of heavy sedative to extinguish the wind, or the products that benefit the essence and fill the marrow from injuring the spleen yang and making the latter lack of source. Therefore, the clinical selection of prescriptions often includes the intention of strengthening the spleen and invigorating the qi, and quenching the wind by transforming phlegm and promoting the circulation of the ligament, either by using Er Chen Tang, Guiding Phlegm Soup, Han Xia Bai Zhu Tian Ma Tang, Tonic Zhong Yi Qi Tang, Purging Phlegm Soup, Ling Gui Zhu Gan Tang, or by supplementing the formula of quenching the liver and quenching the wind and promoting the circulation of the ligament or tonifying the liver and kidneys with products that generate the spleen qi, such as Dang Shen, Bai Zhu, Chen Pi, Fu Ling, Jiang Han Xia, Huang Qi, Shan Yao, Bai Lentil, etc., and adding or subtracting with the evidence. There are various interpretations of the etiology and mechanism of PD, and medical practitioners of all generations have different opinions. In the medical compendium of the Ming dynasty, Lou Ying pointed out that the disease is mostly caused by wind, fire and phlegm; in the section of “all winds and tremors” in the volume 6 of Zhang’s Medical General, Zhang Luyu of the Qing dynasty pointed out that the tremors are caused by wind, fire and phlegm, and should be treated according to the weakness of the spleen and stomach, weakness of the heart, weakness of the heart and shortage of the heart (5) and (6). The common features of PD are old age, physical deterioration, successive deficiencies, improper diet, inappropriate rest, emotional disorders, and excessive strain and injury. The main factors that cause the imbalance of yin and yang in the body, or spleen deficiency and loss of transportation, phlegm and turbidity are generated internally, and heat is turned into heat over time, so the clinical manifestations of PD are mainly involuntary trembling of the limbs. Therefore, the treatment should be to quench the wind and stop spasm as the outline, or use to strengthen the spleen and resolve phlegm, or take the liver to quench the wind, or fill in the marrow, the formula to quench the liver and quench the wind Tang, Hanxia Baijiao Tianma Tang, Tianma hook vine drink, etc., commonly used mother of pearl, raw dragon peony, Tianma, hook vine and other products to quench the wind. In addition, for PD, although there are many variations of evidence, but all have phlegm and blood stasis blocking the meridians of the pathogenesis of the disease, blood deficiency in the veins, then the wind will move within the deficiency, so the prescriptions pay attention to the ancient tradition of “treating the wind first to treat the blood, the wind will be extinguished when the blood is flowing”, according to “qi is the handsome of blood, blood is the mother of qi”. In accordance with the principle of “qi is the commanding force of blood, blood is the mother of qi”, “qi moves blood, and qi stagnation leads to blood stasis”, the prescription is supplemented with qi to benefit the spleen and kidneys or qi to invigorate blood to regulate qi and invigorate blood circulation. Commonly used are Chai Hu, Yu Jin, Chuan Xiong, Citrus aurantium, Hou Pu, Niubizi, Radix Angelicae Sinensis, Radix Paeoniae Alba, Peach kernel, Safflower, Aromatic herb, Di Long, etc. The dosage is mostly between 15 and 30g. The course of PD has a tendency of progressive exacerbation in clinical practice, which can be as long as 8 to 10 years. If not treated properly, patients can become disabled, and it has become one of the top three diseases that cause disability in middle-aged and elderly people. Summing up many years of clinical experience, I found that when PD is stable, phlegm and blood stasis are still present and blood vessels are still penetrating, only because the successive days are all deficient and it is difficult to eliminate the stasis, so the disease mechanism is still present, and the treatment should start from the spleen and kidney; when PD fluctuates, the disease mechanism is mainly based on the good movement of internal wind and the good change of stubborn phlegm, so the treatment should be based on liver wind and phlegm stasis; when PD progresses, it often involves the liver, spleen and kidney due to the interconnection of the three organs, so it is appropriate to treat all three organs together. It is advisable to treat all three organs together. However, it is difficult to accurately grasp the three phases in clinical practice, especially in the fluctuating phase, so it is extremely important to grasp the main evidence and use the prescriptions based on the identification of the three phases. Here the author’s personal experience is that the treatment of phlegm and stasis should be led by strengthening the spleen and resolving phlegm. Because phlegm and stasis block each other, blood vessels are difficult to pass, and stasis does not go because of stubborn phlegm stagnation, phlegm does not dissolve then stagnant blood is difficult to get rid of, so first of all, it is appropriate to strengthen the spleen and dissolve phlegm. At present, Western medicine for PD medical treatment is based on symptomatic treatment, such as neuronal protective agents, compound levodopa preparations, dopamine receptor agonists, catecholamine o-methyltransferase inhibitors, anticholinergic drugs, etc., but all efforts cannot finally solve the progression of PD, especially the decay of drug efficacy after 3-5 years, and the gradual appearance of psychiatric symptoms, gastrointestinal symptoms, switch phenomenon and other serious adverse drug reactions, the The disease deteriorates. In this regard, summarizing the experience of combining Chinese and Western medicine in the treatment of this disease over the years, I believe that in the treatment of PD, Chinese medicine should be used to generate the qi of the spleen and stomach and regulate the qi and blood to slow down the many side effects of Western medicine; to take the rapid and direct effect of Western medicine on the lesion site to make up for the short effect of the slower effect of Chinese medicine to prevent the disease from progressing due to the rapid evolution of the disease and the lack of medication. This is especially suitable for patients with long duration and poor condition. Clinically, we mostly use medication such as medopa, piribedil, amantadine, pramipexole, etc., and adjust the dosage at any time according to the condition, following the principle of “long flow of water, not seeking full effect”, and striving to obtain the best efficacy with the smallest dosage. In the specific treatment, the drugs should be started from small doses, and the dose should be increased slowly according to the changes of the disease, paying attention to the individualization of drug treatment, and combined with the dialectical prescription of Chinese medicine from the spleen theory of treatment, which together can play a role in reducing toxicity and increasing effectiveness. In specific application, for those who have slow progression of disease, who are diagnosed after several months or years of the first disease, and who are under 70 years of age, the compound levodopa preparation can be used late. If the disease is severe and progresses rapidly, and if the patient is over 70 years old, the levodopa complex can be used early. When withdrawing medication, in principle, we should pay attention to the post-up first, which can be done in the order of anticholinergic drugs-propargylamphetamine-amantadine-dopaminergic agonists, and pay attention to the application of western drugs in a measured manner and in an orderly manner. In conclusion, the combined treatment of Chinese and Western medicine for PD is based on maintaining the acquired spleen and stomach, supplementing the spleen and kidney to quench the liver wind, resolving phlegm and supplementing blood stasis, focusing on strengthening the spleen, resolving phlegm and clearing the ligaments, supplementing the liver and Qi, pacifying the liver and quenching the wind, with clear priorities, and paying attention to the reasonable combination of Western medicine treatment, thus achieving more satisfactory clinical results. Typical case Wang, a 66-year-old male, first came to our hospital on March 2, 2007 with the complaints of “trembling hands and slow walking for 3 years”. The patient, a national chef, discovered tremors in his right hand in 2004, and his condition worsened progressively to the point that he could not work normally. The patient’s expression was dull, mask face, hand tremor, tongue tremor, panic gait and other PD evidence, but there were also obesity, whispering and salivation at the corners of the mouth, and the tongue and pulse were fat, dark red, thin and greasy moss, and sunken and stringent pulse, so the Chinese medicine diagnosis belonged to the type of Qi deficiency, blood stasis and phlegm obstruction. The teacher used Si Jun Zi Tang combined with Er Chen Tang to strengthen the spleen, benefit the qi and dissolve phlegm as the main treatment, together with Bai Gan Silkworm and Quan Worm to search the wind and remove the ligaments, and took Yu Jin and Hong Hua to regulate the qi and invigorate the blood and Mother of Pearl to calm the liver and quench the wind.
