Here we will come across a new term: VEGF, which means vascular endothelial growth factor in Chinese. Previous studies have shown that it is related to the occurrence and development of many degenerative lesions in the macula. Therefore, the development and clinical application of anti-VEGF drugs in recent years have brought a boon to patients with macular lesions, and I will give a brief introduction to this upgraded sharpshooter of macular forbidden area below. 1. Introduction: Age-related macular degeneration (AMD), also known as age-related macular degeneration, is an aging change in the structure of the macular region of the retina, which is clinically divided into dry (atrophic) AMD and wet (exudative) AMD, mostly occurring above 45 years of age, with an increasing incidence with age, and is a highly blinding and serious eye disease, the formation of which is closely related to vascular endothelial growth factor ( VEGF is closely related to its formation. Anti-VEGF drugs are a class of biological agents that control the development of AMD by inhibiting the activity of VEGF in the eye. 2. Characteristics: Anti-VEGF drugs are currently used to treat choroidal neovascularization (CNV) of various causes, refractory macular edema and pre-surgical neovascular ophthalmology, commonly wet age-related macular degeneration (AMD), pathological high myopia secondary to macular CNV (PM), central exudative retinal chorioretinopathy, diabetic retinopathy macular edema, and Retinal venous obstructive macular edema, idiopathic choroidal polypoid angiopathy (PCV), etc. 3. Cost: The cost of anti-VEGF drugs is relatively expensive, and there are two types of agents currently available in China: one is the imported razumab, priced at RMB 9,800 per unit; the other is the domestic Compazep, priced at RMB 6,800 per unit. These drugs are fully self-funded and require one per treatment. In addition, there is also the cost of surgery. 4. Treatment process: The treatment method is intravitreal injection (drug injection in the eye), the injection dose is 0.05ml/time, depending on the condition, the interval between each injection is about 1 month, and the regular 3 months is a course of treatment, after which the follow-up is once a month. During the follow-up period, the treatment will be given again according to the change of the disease. 5. Pre-operative preparation: antibiotic eye drops were applied to the operated eye for more than 3 days before treatment, 4~6 times a day. 6. Treatment effect: Treatment with anti-VEGF drugs may significantly improve the patient’s vision, but there may also be cases where the treatment effect is not significant, and there is no guarantee that anti-VEGF drug treatment will be effective for the condition; most patients need repeated treatment to maintain the effect. The effect of treatment depends on the individual condition, and very few patients may be completely ineffective, or even aggravate the ischemic state of the fundus, and in serious cases may have reduced or lost vision. 7, side effects: anti-VEGF drugs need intravitreal injection, so there are certain risks and side effects. The main points are as follows: (1) after treatment, there may be bleeding at the injection site, conjunctival congestion, eye pain, foreign body sensation and other discomfort; (2) after treatment, there may be corneal abrasions, corneal edema and other phenomena; (3) after treatment, there may be an increase in intraocular pressure, black shadows floating, etc.; (4) the most serious risk is intraocular infection, retinal detachment or vitreous hemorrhage, the incidence of which is currently reported to be 0.1%-0.01% (4) the most serious risks are intraocular infection, retinal detachment or vitreous hemorrhage, the incidence of which is currently reported to be 0.1%-0.01%, and once they occur, they need to be treated surgically; (5) accidents due to anesthesia, accidents due to the patient’s age or complications of heart, brain and lung diseases; (6) accidents that cannot be explained or solved by current medical science. 8, post-operative precautions: the day after treatment, please do not touch water and dirt in the operated eye, follow the doctor’s instructions to use antibiotic eye drops, the next day to the hospital to review and then remove the gauze. If you have eye pain and redness, come to the hospital in time. After surgery, pay attention to eye hygiene, do not rub your eyes with your hands, avoid bright light exposure and long time use of eyes, and review again in about 1 month. 9. Combination therapy: Although intraocular injection of anti-VEGF drugs is the first-line treatment, some patients have more stubborn lesions and more complicated conditions, and a single anti-VEGF drug does not achieve the desired effect, and PDT (photodynamic therapy) or other drug therapy can be added according to the condition.
