Migraine is characterized by recurrent attacks that do not heal over a long period of time and belongs to the category of “head wind” in Chinese medicine. Head wind is a disease with the characteristics of unpredictable action and cessation, either on the left or on the right side, with a sudden onset and cessation, and a recurrence of retreating touch after healing. It has the wind phenomenon of good movement and change. The head is the meeting of all the yang, living in the high top position, only the wind evil can reach, according to the evidence to seek the cause, head wind is mostly caused by wind evil. The pathogenesis of migraine is the loss of harmony of the clear orifices and the blockage of the veins. Headache is often caused by wind, which is also the basis for ancient and modern doctors to treat headache with wind medicine. For example, Li Zhongzi, a doctor of the Qing Dynasty, said in “The Essential Reading of the Medical Sect and Headache”, “Headache has many causes, but what is the ancient formula and the use of wind medicine? Above the high top, only the wind can reach. Internal injuries are caused by wind, cold, phlegm, stagnation and heat, and wind and fire are important causative factors, among which wind is the most common causative factor of headache. Wind is a Yang evil that flourishes in spring and is prevalent in all seasons, and belongs to the liver in the organs. The liver vein and the liver vein are located at the top of the brain, on the forehead, under the cheek and around the lip, so the head and face pain is most closely related to the liver. Liver disease is prone to depression, and depression turns into fire, wind and Qi. Fire is a yang evil, and its nature is inflammatory. Whether it is yang Sheng turning into fire, evil depression turning into fire, emotion turning into fire or yin deficiency dragon thunder fire, it can disturb the clear orifices and cause headache. Therefore, fire-heat upward disturbance is an important pathological factor in migraine. Fire is born within, and its disease can involve the heart, liver and stomach, with the liver being the main cause. The head wind belongs to internal injury, and the symptoms of wind-fire, wind-phlegm, wind-cold, wind-phlegm-fire and wind-fire-phlegm-stasis are common. In Jing Yue Quan Shu (Headache), it is said: “In the diagnosis of headache, we should first examine the long and temporary, and secondly identify the surface and the interior, because the temporary pain must be due to evil qi, and the long pain must be accompanied by vital qi. In the case of temporary pain, …… there is the internal evil, the fire of the three yang is incandescent in the inner one, …… treatment should be clear and descending, …… its long term disease, then either hair or healing, or above deficiency in the lower and the false fire multiplied by the then hair …….” Headache caused by fire, clinical to the liver and gallbladder wind and fire on the disturbance for the most common, then the four reverse powder, small Chai Hu Tang, large Chai Hu Tang, scutellaria soup, white head weng Tang, gardenia black bean soup, wind leading soup, Hou’s black powder and so on can be used with the evidence. Insufficient liver blood and liver yang are the common symptoms of migraine. If blood stasis is obstructed and does not pass, it is painful, so Si-wei-san and Gui Zhi Fu Ling Wan are added. Cold stagnation is one of the causes of headache. The cold that causes head wind is the cold of external sensation that stays in the brain for a long time, and the cold of internal smallness that is the result of yang deficiency and yin strength. Cold is a yin evil, and it is easy to hurt yang energy. Whether the external cold stays for a long time, or Yang deficiency cold, clinical signs of Yang deficiency and Yin cold stagnation can be seen. The external cold with Wu Tou Tang, Wu Tou Gui Zhi Tang, Yang deficiency cold with ephedra with seeds and pungent soup, internal cold Wu Zhu Tang available; if the blood deficiency long cold, then Angelica Sinensis four reverse plus Wu Zhu Yu Sheng Jiang Tang is preferred. For Yangming stomach heat attacking upward, head and face pain, or forehead brow bone pain, use Bai Hu Tang with Bai Zhi and Ge Gen; for constipation, foul breath and yellow dry tongue coating, use Da Cheng Qi Tang with Ge Gen; for unpleasant stool and yellow greasy tongue coating, use Yin Chen Artemisia Tang with bitter ginseng and Bai Xian Pi, or use Ge Gen Scutellaria Tang with flavor. Phlegm is a common pathogenic factor in the development of migraine, and the recurrent attacks of migraine that do not heal for a long time are often related to the presence of phlegm in the body. In Danxi Xinfa (Headache), it is said that “Headaches are mostly caused by phlegm, and those who suffer from pain are often suffering from fire.” For headache caused by phlegm turbidity, we can add or subtract Han Xia Hou Pu Tang and Da Chuan Xiong Wan; for headache caused by phlegm heat, we can add or subtract Xiao Shen Che Tang and Da Chuan Xiong Wan; for headache caused by phlegm fire, we can add or subtract Bai Tou Weng Tang and Xiao Shen Che Tang. During the acute attack of migraine, the pathogenesis is mainly based on the blockage of wind, fire, phlegm and cold, while wind, fire, phlegm, stasis and cold are often intertwined with each other, so the treatment often needs to be balanced. In the remission period of head wind, with the decay of the symptoms of wind, fire, phlegm and cold, the dysfunction of the liver, spleen and kidney and other internal organs as well as the symptoms of deficiency can appear. The brain is the sea of marrow, relying on the liver and kidney essence and blood and the spleen and stomach to distill the essence of water and grain, and to distribute qi and blood to moisten. If the spleen is deficient in transporting and transforming, the qi and blood will be deficient. For blood deficiency headache, use Jiao Ai Tang with Chrysanthemum and Tian Ma; for deficiency of kidney essence, use Plus and Minus Fu Pulse Tang with Fructus Lycii, Tortoise Plate and Shou Wu; for deficiency of kidney yang, use Jin Kui Kidney Qi Wan with Chuan Xiong, Hsiang Xin and Tian Ma; for deficiency of qi and blood, use Huang Qi Jian Zhong Tang with Angelica Sinensis and Tian Ma; for deficiency of vegetative body and deficiency of both qi and blood, yin and yang, use Roasted Licorice Tang; for headache with wind evil, use Dioscorea Pill. Migraine has the characteristics of recurrence and remission, therefore, attention should be paid to exploring effective measures to prevent recurrence and to the treatment of the remission period. In the remission period, attention should be paid to treating the root cause, unblocking the emotions and regulating the qi, harmonizing the qi, blood, yin and yang, and also paying attention to the internal ambient evil qi, such as getting rid of blood stasis, phlegm turbidity, depression heat, phlegm dampness and so on. Combined with reasonable care, attacks can be reduced or avoided.
