This is the first time I have taken the time to tell you about it. The hepatitis C as a patient should understand the knowledge points are: 1, the world treatment of hepatitis C program is currently consistent, are the application of alpha interferon combined with ribavirin treatment, recently, a new drug called protease inhibitors will soon be marketed, but this drug or the application of alpha interferon combined with ribavirin on top of the application, and its individual application is not effective. This combined application is only for genotype 1 patients, because, non-genotype 1 people apply interferon combined with ribavirin that can achieve good treatment results! 2, HCV-RNA (hepatitis C virus as RNA virus) can be divided into type 1 and non-type 1 (type 2, 3, 4 – this is the foreign typing method) according to the good or bad treatment effect. Non-genotypes 1 are better treated! On the contrary, genotype 1 (including 1a and 1b) are difficult to treat. 3, do not underestimate the role of ribavirin in the anti-hepatitis C virus, some scholars believe that it should play a 1/3 role in the antiviral efficacy, therefore, as soon as treatment is started, it should be applied in combination with alpha interferon, moreover, the relapse rate after the combination is lower than the application of alpha interferon alone. The dose of ribavirin application should also be noted, it should be 10.6mg/kg, which should be calculated precisely. 4, on the choice of alpha interferon: alpha interferon is currently applied, in Beijing can also be reimbursed, there are three main categories, the first is the long-acting interferon, injected once a week, the main preparations are Pyroxin – pegylated interferon alpha 2a (there are 180ug, 135ug two doses,,, medical insurance can be reimbursed 90%), Pellegrin – pegylated alpha 2b (there are 100ug, 80ug, 50 The second category is the imported short-acting, or ordinary alpha interferon, which is currently commonly used mainly as a multi-dose pen of Gannon, which needs to be injected subcutaneously every other day; the third category is the ordinary interferon produced in China, which also needs to be injected every other day (subcutaneous or intramuscular depending on the instructions), and there are many manufacturers. The advantage of long-acting interferon is that it is easy to use and has the best efficacy for chronic hepatitis C, especially for refractory hepatitis of genotype 1, which is significantly better than the domestic ordinary interferon; the disadvantage is that it is expensive. Firstly, it is very convenient to inject, similar to the Novo pen used by diabetic patients, the needle is small and thin, and the injection is not painful at all, so patients can inject it at home by themselves; secondly, its toxic side effects are milder than long-acting interferon and domestic preparations, especially the effect on blood picture is less (even more so when compared with Pyroxin); thirdly, the price is medium, about half of long-acting interferon (RMB 2400 per month). The third, the price is medium, about half of the long-acting interferon (2,400 yuan per month), but 2-3 times more expensive than domestic interferon, which should be tolerated by the general white-collar class, not to mention that for hepatitis C this drug is covered by health insurance reimbursement! The third category is domestic generic interferon, which can also be used safely for non-type 1 chronic hepatitis C. I have personally cured many patients with the use of long-acting interferon before it was available in China. If the genotyping of hepatitis C cannot be done locally and the patient has limited financial means, you can also choose domestic interferon treatment. 5, about the course of antiviral treatment: combined with the provisions of the 2004 treatment plan for chronic hepatitis C and my personal opinion, the course of antiviral treatment can be mastered as follows: (1) the gene is not type 1, or / and HCV-RNA quantification < 2 × 106 copies / ml: the application of pegylated interferon alpha 2a or 2b, combined with the application of ribavirin (according to kilograms of body weight), the course of treatment for 24 weeks; ordinary interferon should be at least 48 weeks. (2) For those with genotype 1, or/and HCV-RNA quantification <2×106 copies/ml: pegylated interferon α2a or 2b in combination with ribavirin (based on kg body weight) for a course of at least 48 weeks; regular interferon should also be applied for at least 48 weeks, or even for 1.5 years. The specific length of treatment for each patient also needs to be predicted by the doctor in charge of the consultation and treatment through early (4 weeks of treatment), early (12 weeks of treatment), etc., and combined with the specific situation of the patient. 6, about alpha interferon toxic side effects of tolerance: for acute and chronic hepatitis C must be applied interferon for treatment, interferon does have many toxic side effects. But the vast majority of patients are tolerable! Patients must be persistent! Also, the doctor should have a simple, clear introduction to interferon before treatment. During the course of treatment, patients should have regular checkups at the hospital and see their doctor at least once every 3 months! Interferon is usually used for a year before its toxic effects wear off, and an extended course of interferon and ribavirin can help reduce relapses after stopping the drug! For all chronic hepatitis C patients, hepatitis C is a curable disease! If you are unfortunate enough to have hepatitis C, treat it aggressively and you can definitely have a healthy body! In addition, patients who have a history of blood transfusion or surgery for any reason should remember to check for hepatitis C virus and antibodies and ALT after blood transfusion or surgery, because the onset of hepatitis C is very insidious and not easy to detect, only the elevation of ALT is easy to be detected, and, infected with hepatitis C virus, 85% will become chronic, and chronic hepatitis C progresses to cirrhosis and liver cancer much faster than hepatitis B Much faster! Early treatment benefits ah!
