How to treat varicose veins

  Is it a fairy tale to put a “ring” on a blood vessel? It’s not a fairy tale, it’s real! Primary deep vein valve insufficiency is the most common type of venous disease involving blood backflow. At the distal end of the highest pair of valves in the superficial femoral vein, a ring of autologous blood vessels or artificial blood vessels is used to repair the function of the diseased valves, i.e., vascular valve ringing. Our Thoracic and Cardiovascular Surgery Department is the first to successfully perform this procedure in Kaifeng, with high quality vascular surgery techniques for the benefit of the people in the east of Yuandong.  Saphenous varicose vein is a common vascular lesion in the lower extremities, and the epidemiological statistics show that the incidence is about 15%-20%. Individual occupations such as primary and secondary school teachers, surgeons, shopping mall salesmen and manual laborers can be as high as 38%. The main clinical manifestations are superficial varicose veins of lower limbs, soreness and heaviness of affected limbs, dystrophic skin lesions, desquamation, etc., and even long-term skin ulcers of lower limbs, which seriously affect people’s health. Saphenous varicose vein alone accounts for only 16% of blood reflux diseases, while the incidence of primary lower limb deep vein valve insufficiency is the highest, accounting for about 55%. For saphenous varicose veins caused by deep vein valve insufficiency, the use of traditional saphenous vein stripping is highly prone to recurrence. Specialized treatment is especially important for saphenous varicose veins.  The cause of primary lower extremity deep venous valve insufficiency is still unclear, and valve injury may be the main cause. Under the gravitational effect of the blood column, the free edge of the deep vein valve relaxes and sags, and cannot be tightly aligned in the lumen, so it loses its unidirectional opening function, which is the cause of venous regurgitation. The first pair of superficial femoral valve is the strongest venous valve, which is the key valve to maintain the normal hemodynamic state of the deep venous system of the lower extremities, and can resist the reverse pressure of 350-420mmHg, and most patients can achieve good surgical results by treating the first pair of valves only. In most patients, the first pair of valves can be treated. The circumference of the valve is measured below the valve, and the circumference of the vein is taken from the stripped saphenous vein or an artificial vessel, and the vessel wall is split to make a vessel piece 2/3 of the measured circumference of the superficial femoral vein and 1-2 cm wide. The annulus is then fixed to the vein wall with 1 to 2 stitches to avoid slippage, and the valve “ringing” is completed. After surgery, the vein where the diseased valve is located is narrowed to a certain limit so that the free edges of the two opposing valves can be brought together after the valve fossa fills and does not sag and cause a funnel-shaped gap that can lead to backflow of blood.  The vascular valve ringing procedure is simple, safe and easy to master, with satisfactory results, but it must be noted that – the ringing procedure should not be narrowed too little to achieve the surgical effect; at the same time, it should not be over narrowed to cause venous reflux obstruction and lead to thrombosis.
