Bunions are a common condition in foot and ankle surgery and there are various surgical methods depending on the patient’s age, foot condition, physical condition and requirements for treatment results. Orthotics and split-toe pads can be used if the bunion is not severe, but the effect I think is to treat the symptoms but not the root cause. For bunions, the general rule is no pain without surgery. International surgical options: Chevron+Akin or Scarf+Akin, sometimes with an adductor tendon cut, and if there is a problem with the second toe, we also do a second toe capsulorrhaphy or an arthroplasty. Pre-operative examination: generally includes each routine laboratory test, electrocardiogram, positive and lateral films of the foot. Anesthesia costs about 2500 yuan, from the safety point of view, we prefer to choose epidural anesthesia, so that the operation is supervised by an anesthesiologist, a shot in the waist can be, after anesthesia under a tourniquet surgery, the incision is not easy to bleed, the surgical field is clearer, not easy to hurt the vascular nerve tendons, the disadvantage is that the anesthesia fee to be higher. If the patient insists on saving the cost of anesthesia, you can also do it with local anesthesia. I usually do the ankle block anesthesia for the patient myself, and the effect is also good. Hospitalization time: Patients with bunion surgery are usually admitted to the hospital on the second day for various examinations, including ECG, chest X-ray, blood sampling, etc. The surgery is performed on the third day, and the patient can go home 3 days after the surgery if the incision is okay. Post-operative exercise and rehabilitation: During hospitalization, we will provide rehabilitation exercise treatment for patients, and at the same time, we will inform patients of post-operative precautions when they are discharged from the hospital, so that they can carry out subsequent rehabilitation exercises. Epidural anesthesia is usually followed by walking on the ground the day after surgery (because of the anesthesia period), and a cast is usually not required after surgery. You can eat, go to the bathroom, etc. More time is still spent resting, and the amount of activity should be gradually increased. For those who have local anesthesia surgery, we also recommend walking on the next day, mainly to reduce bleeding from the incision caused by premature postoperative activities. After the surgery, we need to wrap bandages for 6 weeks and walk on a flat surface. We used to do “minimally invasive” surgery, but now I prefer to choose the surgery according to the condition. I believe that there are certain indications for minimally invasive surgery, and if there are other toe deformities or significant plantar pain, other procedures should be added. Here I would like to inform you a common sense: we Chinese people often say that it takes 100 days to break a bone, which is actually very true. Therefore, some hospitals say that they will get better immediately after the surgery in order to make advertising campaigns, which I personally think is not very realistic. The general post-operative foot swelling is about 3 – a 6 months, six months to a year will have discomfort, surgery is to improve the symptoms and avoid further aggravation of a kind, rather than after surgery without any discomfort. Of course, some patients do not feel very satisfied with the results after surgery, so they should be prepared for a poor surgical outcome. I often say to people that even a good facelift for an old lady will not restore her beauty to that of her youth, so you should not have more than rational expectations about the surgery. Also after surgery it is recommended to try to wear loose shoes on non-essential occasions, and also to wear five-toed socks, sandals with pinched toes, flip-flops, etc.
