Tear troughs exist due to localized skin folds. While skin folds are a normal physiological structure, the presence of tear troughs is caused by localized connective tissue within the subcutaneous tissue pulling on the skin and thus creating skin folds. The ability of the skin to hold on to the surface of soft tissue is mainly achieved by the pulling of the fibrous connective tissue. Where the fibrous connective tissue pulls on the skin, it may form folds or depressions in the skin, which is why the tear troughs are formed.
