With the change of people’s work and lifestyle, there is a tendency for the onset of lumbar disc herniation to increase and become younger. The age is 18-60 years old and the predisposing factors are degeneration, occupation, smoking, psychological factors, medical injury, physical activity, and cold and obesity. Lumbar disc herniation is difficult to treat and prone to recurrence, and with the passage of time of herniation, most herniations tend to adhere to the dura, which not only aggravates the symptoms, but also causes calcification of the herniation over time, resulting in irreversibility of conservative treatment of the herniation. The injection of Chuanxiongzin directly into the epidural space through the sacral canal can effectively prevent the adhesion of the herniated collagen to the dura, which not only can relieve the symptoms but also can effectively slow down the process of herniated calcification. Chuanxiongzine, chemically known as 2,3,5,6 tetramethylpyrazine, or tetramethylpyrazine, is an alkaloid contained in the traditional Chinese medicine of Chuanxiong, which has the effect of activating blood circulation and resolving blood stasis, as well as regulating qi and relieving wind and pain. The main pharmacological effects of Chuanxiong Ligusticum are: 1) vasodilation; 2) mild hypotension; 3) improving tissue microcirculation and tissue perfusion; 4) inhibiting platelet adhesion and aggregation and thrombosis; 5) inhibiting smooth muscle cell and fibroblast proliferation; 6) regulating lipid metabolism and anti-lipid peroxidation; 7) regulating immune effects. Anti-tissue fibrosis and inhibition of platelet adhesion and aggregation is an important aspect of the pharmacological effects of chuanxiongzin, which has certain therapeutic effects on both pulmonary fibrosis and hepatic fibrosis. When cultures containing chuanxiongzin were added to proliferating scar fibroblasts, the results showed that chuanxiongzin significantly inhibited the proliferation of scar fibroblasts. The mechanism of action involved anti-inflammatory response, improvement of tissue blood supply, inhibition of fibroblast nuclear division and proliferation, and reduction of fibroblast DNA and collagen synthesis. It also has a regulatory effect on the expression of type I and III pre-collagen m RNA in fibroblasts. Currently, Chuanxiongzin has been studied in cardiovascular and antitumor areas, but little research has been reported in the literature on the prevention of epidural scarring.
