Lung cancer is growing rapidly in incidence and mortality, and is one of the most threatening malignancies to human health and life. Currently non-small cell lung cancer (NSCLC), i.e. squamous carcinoma, adenocarcinoma and large cell undifferentiated carcinoma, accounts for about 70% – 80% of all cases of lung cancer. Early and intermediate stage NSCLC should be considered for surgery first, while most cases are advanced patients, mostly stage III-IV patients, who have lost the opportunity of surgery, and chemotherapy is the most common treatment method. The biological characteristics of non-small cell lung cancer determine that it is not sensitive to radiotherapy and chemotherapy, and although it has certain efficacy, it rarely has ideal results. Clinical efficacy has improved in recent years, but the improvement of chemotherapy efficiency has not improved the long-term survival of patients, and the median survival of advanced lung cancer is still about 8 months. Recent data show that despite the great progress in the treatment of lung cancer, mainly in the application of new chemotherapeutic drugs and linear gas pedals, the five-year survival rate of lung cancer has only increased from 13.9% at that time to 16.9% in 30 years. Therefore, how to improve the efficacy of chemotherapy, reduce the toxic side effects of chemotherapy, improve the quality of life and prolong the survival is a very difficult problem we must face. The formation of lung cancer is mainly due to the deficiency of positive qi, the dysfunction of internal organs, the entry of evil toxins, the stagnation of lung qi, the loss of the function of declaring and descending, the lack of smooth qi flow, the stagnation of qi and blood stasis, the obstruction of the channels and veins, the unfavorable distribution of fluid, the congestion of phlegm, and the intersection of phlegm and stasis, thus forming a lump. Lung cancer is a systemic disease, but the tumor in the lung is only a local manifestation of the whole body, which is caused by the deficiency and the deficiency causes the reality; the whole body is deficiency and the local is reality; the deficiency is the root of the disease and the reality is the symptom of the disease. In our study, we believe that the deficiency of lung cancer has Yin deficiency and Qi deficiency as the fundamental pathological changes. Lung cancer is mostly caused by middle-aged and elderly patients, who are frail and sickly, and have been suffering from long-standing illness and strain, and are already suffering from deficiency of lung qi and deficiency of yin; external evil attacks the lung by taking advantage of the deficiency, and the heat and toxicity will further injure the yin and fluid of the lung. At present, there are many kinds of western medical treatments for lung cancer, but each of them has a great impact on the normal functions of human body, and they are all at the cost of damaging the normal functions of the body. Some patients even have to give up the treatment plan of Western medicine because of poor tolerance. Clinical studies in Chinese medicine have shown that deficiency, phlegm, blood stasis and blood stasis are the main types of advanced lung cancer. Accordingly, the “double attack and double adjustment” method was proposed in clinical practice to treat advanced lung cancer. This method is based on the basic theory of traditional Chinese medicine of the motherland, under the treatment principle of “eliminating the evil without harming the righteousness”, combined with modern medicine and the treatment rules of Chinese medicine and the actual clinical situation for many years, Professor Jiang Jiakang concluded the effective method of combining Chinese and Western medicine for the comprehensive treatment of advanced lung cancer. The method of “double attack and double adjustment” refers to: one attack is to use the effect of Chinese medicine to attack and expel phlegm and stasis to improve the state of phlegm and stasis caused by the imbalance of yin and yang in the patient’s body; one adjustment is to further adjust the function of internal organs of the patient on the basis of one attack of phlegm and stasis to achieve the purpose of “prevention before disease”. The purpose is to use the normal function of the body to inhibit the development of tumor cells; the second attack is to use modern medical radiation and chemotherapy to further destroy tumor cells or reduce the aggressiveness of tumor cells; the second adjustment is to use Chinese medicine to regulate the qi and blood function of the body and improve the symptoms of qi and yin deficiency caused by radiation and chemotherapy, so as to achieve the state of “yin and yang secret, spirit and even”. The second adjustment is to regulate the body’s qi and blood functions to improve the symptoms of qi and yin deficiency caused by radiotherapy, so as to achieve the state of “yin and yang secretion, spirit and even”. Radiotherapy plays an important role in the comprehensive treatment of advanced lung cancer, and is a proven treatment method. However, while it inhibits and kills tumor cells, it also brings reversible or irreversible damage to normal tissues and functions of the body, which in turn affects its efficacy and reduces the survival quality of tumor patients. Therefore, the search for drugs and methods that can reduce the toxicity and increase the effectiveness of chemotherapeutic drugs and their rational application is still an important issue in the process of lung cancer chemotherapy. Chinese medicine has a significant role in reducing the toxicity and increasing the efficacy of chemotherapy drugs, and the combination of Chinese medicine with chemotherapy is highly valued and widely used in the treatment of advanced non-small cell lung cancer. After synthesizing domestic and personal clinical research experience, Prof. Jiang Jiakang found that the most common type of advanced lung cancer is Qi-Yin deficiency, accounting for about 83% of the cases. Lung cancer starts from the deficiency of qi, and once the disease develops, various etiological factors and pathological products will further injure the qi, and the deficiency will be further increased. The development process of lung cancer is essentially a process of the struggle between evil and positive. Therefore, the principle of supporting the righteousness should be applied to the whole process of lung cancer treatment. Based on the physiopathological characteristics of lung, the deficiency of lung mainly lies in the deficiency of qi and yin. In clinical practice, Prof. Jiang Jiakang has developed a series of Chinese herbal prescriptions, which are used flexibly in the treatment of each type of deficiency, based on the identification of the disease and the examination of the evidence, and have achieved good results in clinical treatment. The “double attack and double regulation” method not only effectively controls the growth of local tumor, but also improves the body’s own ability to resist tumor and reduces the dependence on chemotherapy. After clinical and experimental studies on the treatment of tumors in recent years, it has been shown that the main mechanisms of Chinese medicine in the treatment of lung cancer are enhancing immune function, inhibiting tumor cell proliferation, inducing tumor cell differentiation and apoptosis, sensitizing effect of radiotherapy and reducing toxicity of chemotherapy. Moreover, the efficacy of TCM treatment in improving the survival quality, prolonging the survival time with tumor, improving clinical symptoms and increasing the efficacy and reducing the toxicity is confirmed. In addition, Chinese medicine controls tumor metastasis: metastasis is the main reason for the failure of lung cancer treatment and an important subject of clinical research at present, which is a key factor affecting the prognosis of tumor patients. Preventing further metastasis of lung cancer is a hot spot in the treatment of lung cancer in Chinese medicine. According to Prof. Jiang Jiakang, the idea of prevention before illness and prevention after illness is particularly important in the prevention and control of lung cancer metastasis. He believes that deficiency, phlegm and stasis are the main causes of lung cancer metastasis, and the key is to cultivate and consolidate the qi of the organ before metastasis occurs to prevent it from metastasizing to other organs. The “double attack and double adjustment” method has been proved to be the first choice of treatment for patients with mid- to late-stage lung cancer, as it has satisfactory effects in helping to consolidate the root, removing phlegm and stasis, enhancing the body’s resistance, improving the quality of life and prolonging the survival of patients with advanced cancer.
