Not only was this incomprehensible to ordinary people, but it also seriously affected her daily work and she had to resign. Through careful questioning of medical history, we learned that the patient’s face was originally suffering from chloasma, and in recent years she often went to beauty salons to apply the so-called special spot whitening products to remove spots, and gradually the above symptoms appeared on the ground. This situation is most likely due to the addition of glucocorticoids in the external blemish whitening substances, resulting in “hormone dependent dermatitis”. Later, the patient was cured by taking internal herbal medicine for several months to nourish Yin, clear heat and activate blood to remove spots, and treated with “OPT” strong pulse light for several times. Nowadays, some unscrupulous businesses often illegally add hormones or heavy metals such as mercury and lead to whitening cosmetics in order to pursue the so-called “immediate” effect of removing spots and attract customers. Although topical hormones have certain curative effect on melasma, there are many side effects, so it is not recommended to add hormones in cosmetics to increase the whitening effect. Inappropriate topical application of hormones on the face will cause capillary dilation of facial skin, resulting in redness and burning of skin congestion. The epidermal stratum corneum becomes thin, the skin barrier is lost, and skin moisture is easily lost causing dehydration and dry skin. Topical hormones can also cause facial papules, pustules, or coarse hair, or skin atrophy, etc. Cosmetics containing lead and mercury are also a beauty trap. Long-term use of lead and mercury can also cause chronic lead and mercury poisoning, and even life-threatening. The occurrence of melasma is mainly related to sunlight, estrogen and other factors. The influence of ultraviolet rays and estrogen causes the activity of tyrosinase to increase, which stimulates melanocytes to produce too much melanin and causes pigmentation. For the treatment of melasma, I advocate a combination of Chinese and Western medicine to slowly regulate the treatment. At the same time, I caution patients not to blindly pursue “fast spot removal” and use some inappropriate whitening products or other drugs. You can take Chinese herbal medicines to nourish blood, invigorate blood and tonify kidneys, and you can also use decoction of some herbal medicines to invigorate blood and remove spots. In recent years, it has been found that tranexamic acid has a better therapeutic effect on melasma when taken internally. On this basis, the Department of Dermatology of Jiangsu Provincial Hospital of Integrative Medicine has innovatively treated melasma with needle-free micro-injection method and achieved better therapeutic effect. For “hormone dependent dermatitis” caused by inappropriate topical application of hormones on the face, I believe it is due to “Yin deficiency and blood stasis” from the perspective of Chinese medicine, and advocate the use of “nourishing Yin and invigorating blood”. After the symptoms improve significantly, I will use strong pulsed light treatment to close the proliferated and dilated capillaries. The skin has become “dependent” on hormones, just like a drug addict who has become “addicted”, and can only be quit slowly. The “peachy face” is the pursuit of many women who love beauty. Some chronic diseases such as menstrual disorders, dysmenorrhea and liver disease should be actively treated. Since birth control pills can cause chloasma, it is important to weigh the pros and cons whether to take them or not.
