How to treat lacunar cerebral infarction

For the treatment of lacunar cerebral infarction, if the onset of acute cerebral infarction is within 4.5 hours, intravenous thrombolysis with alteplase can be given; beyond 4.5 hours, the main treatment is oral aspirin antiplatelet aggregation. If aspirin is not tolerated, oral clopidogrel can also be given, along with oral statins, which are commonly used clinically as atorvastatin and simvastatin. If the patient has a history of hypertension or diabetes mellitus, it should be actively given for regulation. Commonly used blood pressure lowering drugs, mainly amlodipine and irbesartan, but also valsartan and enalapril can be applied. If the patient has a history of diabetes, metformin is preferred orally, and insulin can also be applied to lower glucose therapy if necessary, so that the patient’s glycated hemoglobin can be controlled to about six as much as possible. If the patient has hyperhomocysteinemia, he should be actively given regulation, oral vitamin B6, folic acid tablets, and methylcobalamin. Also if the patient has severe somatic signs and symptoms, acupuncture treatment and rehabilitation can be administered.
