Childhood leukemia risk assessment

  I. Acute lymphoblastic leukemia (ALL) in children
  According to the criteria proposed by the Hematology Group of the Pediatrics Branch of the Chinese Medical Association (2006), ALL is classified into 3 types.
  HR-ALL (high-risk acute gonorrhea)
  MR-ALL (medium-risk acute gonorrhea)
  LR-ALL (low-risk acute gonorrhea)
  HR-ALL (those with 1 or more of the following) can be diagnosed
  1. Infants ≤12 months of age.
  2. Peripheral blood leukocyte count ≥100×109/L at the time of initial diagnosis.
  3, Chromosomal karyotype and fusion gene are.
  t(9; 22), BCR-ABL fusion gene (+).
  t(4; 11), MLL-AF4 fusion gene (+), or the presence of other MLL gene rearrangements.
  4. Those with poor response to early treatment (those with 1 or 2 of the following)
  (1) Prednisone 60mg/m2 daily for 7 days of induction, peripheral blood leukemic cells >1×109/L on day 8).
  (2) Bone marrow leukemic cells >5% on day 19 of chemotherapy.
  Be careful of treatment failure (complete remission not achieved at 6 weeks of chemotherapy) during primary treatment induction remission therapy.
  MR-ALL (those with 1 or more of the following)
  1.≥10 years old.
  2.Peripheral blood leukocyte count ≥50×109/L at the time of initial diagnosis.
  3. Central nervous system leukemia (CNSL) and/or testicular leukemia (TL) at the time of initial diagnosis.
  4, Hypodiploidy with chromosome number ≤45.
  5. Any chromosomal or genetic abnormality other than t(9;22), t(4;11), such as the presence of E2A-PBX1(+).
  6, T-cell leukemia.
  LR-ALL
  1, 1 year old and 10 years old.
  2, Peripheral blood leukocyte count 50×109/L at initial diagnosis.
  3, No CNS leukemia and/or testicular leukemia at the time of initial diagnosis.
  4. No chromosomal, genetic abnormalities.
  5. Good response to early treatment.
  6. Initial treatment with induction chemotherapy and complete remission was obtained in 6 weeks.
  II. Childhood acute myeloid leukemia (AML)
  According to the criteria proposed by the Hematology Group of the Pediatrics Branch of the Chinese Medical Association (2006) AML is also divided into 3 types.
  HR-AML
  MR-AML
  LR-AML
  HR-AML (those with 1 or more of the following)
  1. Infants ≤12 months of age.
  2. Peripheral blood leukocyte count ≥100×109/L at initial diagnosis.
  3. Chromosomal karyotype is: 7.
  4. MDS-AML.
  5, No remission for 1 course of standard regimen.
  LR-AML (LR-AML is described here first)
  1, APL (M3).
  2, M2b, M4Eo.
  3, AML (with inv16).
  MR-AML
  Excluding HR-AML and LR-AML, all AML.
