I. Initial consultation
In December 2021, the patient came to our hospital and reported that he had a red flat wart on his hand 3 months ago, which he then picked and scratched with his nails.
On examination, multiple scattered red hyperplastic tissues with flattened tips and well-defined margins were seen on the right dorsum of the patient’s hand. Mild scratch marks could be seen on the back of the hand. The patient’s description and clinical symptoms were considered to be lichen planus. After communication with the patient, the patient agreed to a pedestrian papillomavirus genotyping test, which showed positive results for HPV low-risk types 3 and 11. The results were consistent with the diagnosis of lichen planus.
Second, the treatment process
The patient’s warts had been present for 3 months and had a tendency to gradually expand and increase. The patient agreed to undergo subcutaneous implantation of flat warts with a topical anti-HPV bioprotein gel dressing. The newly developed flat warts on the back of the hand were disinfected with iodophor and then anesthetized with lidocaine hydrochloride injection, followed by removal of the entire wart with a sharp knife and sterile gauze wrapping after disinfection with iodophor. This treatment allows the body to produce the appropriate antibodies to the HPV virus, causing the damage on the surface of the skin to gradually shrink and fade. The patient will take back the medication after the surgery, and after understanding the method of medication, nod that he will go home for home treatment and apply the medication on time.
III. Treatment effect
After 15 days of treatment, the patient returned by telephone and said that the stitches were removed at the community hospital 7 days after the subcutaneous warts implantation, and there was no local redness or swelling. As of the date of the telephone call, the right hand part of the scattered flat warts has gradually flattened, the color basically returned to normal. The patient was instructed to continue using the medication and to follow up any time there is a problem, not to stop the medication too early to avoid recurrence.

I’m glad that the patient’s warts were controlled after surgery and medication, and the skin on the hands gradually recovered. The patient needs to be aware that because of the wounds at the subcutaneous implantation of flat warts, it is recommended not to get wet within 3 days to avoid the formation of infection. It is recommended not to exercise excessively until the stitches are removed to avoid breaking the stitches and affecting the local recovery. Drug application needs to be applied after washing the hand and rubbing it lightly to promote drug absorption in order to achieve the corresponding effect as soon as possible. Before the disease is completely cured, do not have intimate contact with others in the affected area to avoid infection.
