Physiological penile scrotal pain is a group of physiological phenomena that cause penile and scrotal pain due to any reason such as mental or mechanical stimulation, resulting in repeated or prolonged erection of the penis. Etiology: This syndrome mostly occurs in erotic places, including visual, auditory or excessive intimacy with sexual partners, fondling and other reasons, so that the penis repeatedly or continuous erection for too long, and not to reach orgasm, the body is always in a state of sexual excitement. It causes the external genitalia and pelvis to be depressed with blood for a long time, resulting in local tissue hypoxia. Due to the special physiological state of hypoxia, its energy source is mainly dependent on glycolysis, which eventually produces a large amount of lactic acid and accumulates in the tissues. The mechanism of its occurrence is the same as that of overexertion, which finally produces acidity, swelling, pain and swelling in a certain muscle group of the body. Clinical manifestations: The syndrome is not obvious in the state of sexual excitement, but gradually worsens afterwards. It manifests as penile soreness, testicular pain, mild swelling of the epididymis, and tenderness. The pain can be radiated to the lower abdomen, especially when standing up. A small number of people may have perineal swelling and pain, often with a feeling of stool. These symptoms can last for several hours until they are relieved after sleep. In general, the symptoms can disappear or are only sore and swollen if the person sleeps for more than 6 hours. Differential diagnosis: This condition should be distinguished from orchitis or epididymitis. It occurs after a long erection and lasts for several hours and is relieved after sleep. Treatment: No special treatment is needed for this condition. Sedative drugs can be given to those with severe symptoms.
